摘要
目的:探讨幽门螺杆菌(Hp)阳性的原发免疫性血小板减少症(ITP)患者T淋巴细胞凋亡及其意义。方法:收集我院48例Hp阳性的ITP患者,给予三联抗Hp治疗,检测治疗前后外周血T淋巴细胞凋亡率、血小板计数(PLT)、Caspase-3和bcl-2表达水平。结果:48例ITP患者给予抗Hp治疗后,43例(89.6%)转阴,5例未转阴。43例Hp转阴患者中30例疗效评价为有效(显效+良效+进步),治疗后PLT[(86.52±9.58)×109/L∶(15.84±5.48)×109/L]、T淋巴细胞凋亡率[(3.79±0.57)%∶(1.95±0.54)%]、Caspase-3表达率[(28.26±6.62)%∶(13.59±2.35)%]较治疗前均明显升高(均P<0.05),而bcl-2表达率明显下降[(32.51±5.75)%∶(78.45±6.21)%,P<0.05]。其余18例(包括13例Hp转阴患者和5例Hp未转阴患者)疗效评价为无效,治疗后PLT、T淋巴细胞凋亡率、Caspase-3和bcl-2表达水平较治疗前均差异无统计学意义。结论:Hp感染导致慢性炎症刺激,从而引起ITP患者T淋巴细胞凋亡异常,免疫功能紊乱,PLT持续低下,因而清除Hp感染成为ITP的一种治疗手段。
Objective:To explore the role of T lymphocyte in the pathogenetic mechanism of primary immune thrombocytopenia (ITP) infected with Helicobacter pylori (Hp). Method:The platelet count (PLT), apoptosis rate of T lymphocytes, expressions of Caspase-3 and bcl-2 protein on T lymphocytes of peripheral monoculear cells from 48 ITP patients infected with Hp were determined by flow cytometry before and after Hp-eradication. Result:PLT, the apoptosis rate of T lymphocytes and the expression of Caspase-3 increased, but bcl-2 decreased in 30 patients (all P〈0.05). In 18 patients, there was no significant difference in PLT, apoptosis rate of T lymphocytes, expres sions of Caspase-3 and bcl-2. Conclusion:The inhibition of T lymphocytes apoptosis in peripheral monoculear cells is possibly related to the pathophysiology of ITP infected with Hp.
