营销人员声嘶患者喉镜观察及嗓音主客观评估 被引量:5

摘要 目的分析营销人员声嘶患者喉镜特征及嗓音主客观评估结果,探讨嗓音主客观检测方法对评估其嗓音损害程度的价值。方法对80例营销人员声嘶患者进行喉镜检查、嗓音障碍指数(voice handicap index,VHI)简化中文版(VHI-13)量表评分、嗓音GRBAS主观听感知评估及嗓音声学检测,以60例教师声嘶患者及80例正常成人作为对照组,比较其结果。结果营销人员声嘶患者喉镜检查主要表现为慢性喉炎、声带小结、声带息肉等声带良性病变,与教师声嘶患者比较无统计学差异。与正常对照组相比较,营销人员及教师声嘶患者的VHI总分增高,总嘶哑度G主要为轻中度异常,基频微扰(jitter)及振幅微扰(shimmer)均值增高,最长发声时间(maxi-mum phonation time,MPT)和发声障碍指数(dysphonia severity index,DSI)均值降低,差异均有统计学意义(P<0.01);而营销人员与教师声嘶患者相比较,各嗓音主客观参数均无统计学差异。结论营销人员与教师声嘶患者的喉部病变及嗓音损害特征基本一致,喉镜均主要表现为声带良性病变,总嘶哑度G均轻中度异常。嗓音主客观评估可定量评估嗓音损害严重程度。 Objective To analyze and compare the laryngoscopic characteristics and the results of the subjective and objective voice assessments between marketing personnel and teacher patients with hoarseness,and to investigate the value of subjective and objective voice assessments in evaluating the voice damage degree of occupational voice patients.Methods Eighty marketing people,60teacher patients with hoarseness and 80normal voice people were tested by laryngoscopic observation,the simplified Chinese version of voice handicap index(VHI-13),GRBAS perceptual evaluation and acoustic analysis.The differences of laryngoscopy and voice assessments between marketing patients and controls were compared.Results The laryngoscopy of marketing and teacher patients mainly presented vocal cords benign lesions.There were no statistical differences in the constituent ratio of vocal cord lesions between marketing and teacher groups.The total VHI score of marketing patients was increased,the total hoarse degrees(G)mainly showed mild to moderate abnormalities and the means of jitter and shimmer of marketing and teachers patients were significantly higher than those of normal controls respectively and the means of MPT(maximum phonation time)and DSI(dysphonia severity index)were significantly lower(P0.01),though there were no statistically differences of voice parameters between two patient groups.The acoustic parameters of marketing patients were noticeably associated with the score on Grade.Conclusion The laryngeal lesions and voice impairment characteristic of marketing patients with hoarseness were essentially consistent with those of teachers,the laryngoscope of those patients mainly manifest vocal benign lesions and the voice function show mild to moderate abnormal.The subjective and objective voice assessments can evaluate the voice damage degree of voice function for occupational hoarseness.
