摘要
目的 探讨缺血再灌流损伤脊髓的病理变化特征及其意义。方法 用 30只成年家兔 ,随机均分为正常组、对照组、缺血 30分钟组、缺血 6 0分钟组和缺血 90分钟组 ,以选择性腰动脉阻断法制备腰髓缺血再灌流损伤模型 ,术后 1、3、7、14天时 ,取腰髓进行HE和Holmes还原银染色以观察其病理学改变。结果 (1)受损脊髓组织早期即发生出血、水肿、变性坏死 ,伤后 7天时可见明显的炎性细胞浸润 ,伤后 14天炎性反应减轻 ;(2 )脊髓灰质的病理损害明显重于白质 ;(3)再灌流脊髓病理损伤程度依次为缺血 90分钟组 >缺血 6 0分钟组 >缺血 30分钟组 >对照组 >正常组。结论 缺血时间越长 ,再灌流脊髓病理损害越重 ,表现为急性进行性。
? Objective To investigate the pathological characters of spinal cord after ischemic and reperfusional injury and its significance.Methods The model of ischemic and reperfusional injury of spinal cord was created by the selective occlusion of lumbar artery in 30 adult rabbits,which were randomly divided into normal group,control group,ischemia 30min group,ischemia 60min and 90min group.After 1,3,7 and 14 days of reperfusion,the pathological changes of lumbar segmental spinal cord were observed with the HE staining and Holmes's argyrophilic staining.Results (1) Hemorrhage,swelling and degeneration of the lumbar spinal cord tissue occurred in the early phase of reperfusion,the remarkable inflammatory cells infiltration was observed at the 7th day of reperfusion,and the inflammatory reaction was lessened at the 14th day of reperfusion.(2) The pathological damage to the gray matter of spinal cord was more serious than that of the white matter.(3) The extent of pathological damage in the lumbar spinal cord after reperfusion was in the order of ischemia 90min group>ischemia 60min group>ischemia 30min group>the control group>the normal group.Conclusion The longer the ischemic time,the more severe the pathological damage of spinal cord tissue after reperfusional injury is,which is characterized by acute,progressive,extensive degeneration and necrosis. 〔
