摘要
目的评价R2*值术前划分透明细胞肾癌(CRCC)病理级别的可行性。方法回顾性分析26例CRCC患者的血氧水平依赖MRI结果,测量肿瘤实质部分的R2*值。使用组织学核分级法将所有肿瘤分级,在此基础上将所有患者分成低级别和高级别两个亚组。比较不同级别CRCC之间及高低级别组CRCC之间R2*值的差异。利用受试者工作特征曲线分析确定R2*值鉴别高低级别CRCC的阈值。R2*值与高低级别组CRCC之间行相关性分析。结果低级别组(Ⅰ+Ⅱ级)17例,高级别组(Ⅲ+Ⅳ级)9例。各级别CRCC之间R2*值存在明显差异(P=0.001)。高级别组R2*值明显高于低级别组(P=0.003)。R2*值为31.87 s-1时,敏感性为78%,特异性达到100%。R2*值与病理级别直接相关(P=0.002)。结论 R2*值作为一种无创的评价指标有助于CRCC的病理核分级。
Objective To assess whether R2^* values can be used to determine the nuclear grade of clear cell renal cell carcinomas(CRCC).Methods Blood oxygen level-dependent magnetic resonance imaging(BOLD-MRI) of 26 patients with pathologically-proven CRCCs was retrospectively assessed.R2^* values were determined for the solid components of CRCC lesions.Histological nuclear grade was determined in each lesion.All patients were divided into low-and high-grade groups.R2^* values were compared between different grades and between low-and high-grade groups.Receiver operating characteristic curves were drawn to establish the cut-off point for R2^* values.The correlation between R2^* value and pathological grade was assessed.Results Low-grade group(Grade Ⅰ+Ⅱ) contains 17 cases and high-grade group(Grade Ⅲ+Ⅳ) contains 9 cases.Significant difference was seen between different grade groups(P=0.001).R2^* values of the high-grade group were higher than the low-grade group(P=0.003).A sensitivity of 78% and a specificity of 100% were achieved with a cut-off of 31.87 s-1.R2^* values directly correlated with pathological grades(P=0.002).Conclusion R2^* values of CRCCs could be employed as a non-invasive biomarker to help classifying the nuclear grade of CRCC.
