摘要
目的探讨单纯全麻及全麻复合腰一硬联合麻醉在腹腔镜下行子宫切除术中应用的安全性及有效性。方法将2008～2010年我院采取单纯全麻进行子宫切除术的60例患者作为对照组，选择2010～2012年采取全麻复合腰一硬联合麻醉行子宫切除的60例患者为观察组，在术中对子宫松弛度进行评级，并记录血液动力学、全麻用药量、手术时间以及患者术毕苏醒时间和术后躁动的情况。结果观察组患者子宫松弛度明显优于对照组（P〈0．01），观察组的手术时间、全麻用药量、术毕苏醒时间均低于对照组（P〈0．01）。结论全麻复合腰一硬联合麻醉在腹腔镜子宫切除术中可使得子宫松弛满足手术要求，减少手术时间，血流动力学稳定，是一种安全可靠的麻醉方式。
Objective To discuss the safety and efficiency of general anesthesia and general anesthesia combined with spinal-epidural anesthesia in laparoscopic hysterectomy application. Methods 60 cases of patients who were admitted into our hospital from 2008 to 2010 for hysterectomy by general anesthesia were selected as control group,meanwhile,another 60 cases of patients using general anesthesia combined with spinal-epidural anesthesia were selected as observation group. Grade evaluation of hysteranesis was conducted during operation,haemodynamics,dosage of general anesthesia,time of operation,recovery time and dysphoria after operation were recorded. Results The hysteranesis of observation group was superior to control group （P 〈 0.01）;the time of operation,dosage of general anesthesia,recovery time were lower than control group（P 〈 0.01）. Conclusion General anesthesia combined with spinal-epidural anesthesia in laparoscopic hysterectomy can make hysteranesis meet the requirements of operation,reduce time of operation,stabilize haemodynamics,it is a safe and reliable anesthesia method.
