免散瞳眼底照相筛查不同分期糖尿病视网膜病变效果的临床研究 被引量:13

摘要 目的探讨免散瞳眼底照相在不同分期为阈值时筛查糖尿病视网膜病变(DR)的价值。方法 T2DM患者180例(357眼)进行免散瞳眼底照相、直接眼底镜检查及眼底荧光血管造影(FFA)检查。结果免散瞳眼底照相在以中、轻度非增殖期糖尿病视网膜病变(NPDR)及以上分期为阈值的检查中,敏感性均>85%,特异性>95%,Kappa>0.8。讨论免散瞳眼底照相是筛查早期DR的一种优化方法。 Objective To evaluate the value of nonmydriatic fundus photography in screening different severity of diabetic retinopathy. Methods One hundred and eighty T2DM patients (357 eyes) were enrolled in the trial. Nonmydriatic digital fundus photography, fundus fluorescence angiography (FFA) and direct funduscopy with pupils dilation were carried out for all patients. Then the data of three examination methods was compared by statistical analysis to evaluate the value of nonmydriatic digital fundus photography. Results In comparison with FFA, the fundus photography gave a sensitivity of 78.6%, a specificity of 96.8%, and a kappa statistic of 0.857 in using mild nonproliferative diabetic retinopathy (NPDR) as a threshold, and showed a sensitivity of 85% or higher, a specificity of 95% or higher, and a kappa statistic of 0.8 or higher in using moderate or severe NPDR as a threshold. Conclusion Nonmydriatic fundus photography is feasible in screening early diabetic retinopathy, especially in using moderate or severe NPDR as a threshold.
