151株淋病奈瑟菌的耐药性分析 被引量:10

Antibiotics resistance of 151 strains of Neisseria gonorrhoeae

摘要 目的检测临床分离的淋病奈瑟菌(NG)耐药性,以了解NG的耐药现状,为合理使用抗菌药物提供科学依据。方法对分离自2009年1月-2011年12月在医院就诊的淋病疑似患者宫颈分泌物NG进行药敏试验;药敏试验采用K-B法,药敏纸片选用5类7种常用抗菌药物,均为英国Oxoid公司产品;β-内酰胺酶测定采用产色头孢硝噻吩纸片法。结果 151株NG对大观霉素的耐药率最低为6.0%,其次是第二、三代头孢菌素,耐药率为13.2%~17.2%;对青霉素、环丙沙星和四环素的耐药率最高,分别为84.8%、78.8%和72.8%;耐药模式以3耐、2耐的构成比最高,分别占39.7%、32.4%;全敏感和全耐药的仅占6.0%和1.3%。结论大观霉素和第三代头孢菌素可作为淋病治疗的首选药物,青霉素类、喹诺酮类和四环素类药物因耐药率太高,已不适合作为一线药物使用;鉴于NG的耐药性变化较快,应加强细菌的培养和药敏试验,根据试验结果合理选用抗菌药物,以减缓NG耐药性的发生和传播。 OBJECTIVE To investigate the drug resistance of clinical isolates of Neisseria gonorrhoeae (NG) so as to provide scientific evidence for the rational use of antibiotics. METHODS From Jan 2009 to Dec 2011, totally 151 strains of NG were isolated from the cervical secretions of patients suspected with gonorrhea infections. The drug susceptibility testing was performed by K-B method and there were 5 types, 7 kinds of commonly used antibacterial drugs (British Oxoid) tested, the β-lactamase was determined by nitrocefin-disk. RESULTS The results suggested that of the 151 strains of NG islated the resistance rate of spectinomycin was the lowest (6.0 %), followed by the second and third generation cephalosporins (13.2 %-17.2 %), the drug resistance rates to penicillin, ciprofloxacin and tetracycline were the highest, which were 84.8%, 78.8% ,and 72.8%, the straius which were resisitant to 3 or 2 kiuds of autibiotics had the highest porprotiou that accounted for 39. 7% and 32. 4% respectively. The antibiotics that were all sensitive and the antibiotics that were all resistant only accounted for 6.0 % and 1.3 %, respectively. CONCLUSION Spectinomycin and third-generation cephalosporins can be recommended for the treatment of gonorrhea. Penicillins, quinolones, and tetracyclines are not suitable for the first-line drugs for the high resistance rates. For the rapid change of resistance, it is necessary to strengthen the bacterial culture and the drug susceptibility testing and reasonably use antibiotics based on the testing result so as to prevent the emergence and spread of drug resistance of NG.
