摘要
【目的】观察大承气汤联合血必净注射液治疗急性重症胰腺炎(SAP)的疗效。【方法】将71例SAP患者随机分为观察组36例和对照组(西医组)35例。对照组给予西医常规保守治疗,观察组在对照组治疗的基础上,加用鼻饲或灌肠大承气汤及静脉滴注血必净注射液,观察2组临床疗效、胃肠功能恢复时间,以及血清肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、IL-8水平变化。【结果】观察组的临床总疗效显著优于对照组,患者腹痛、腹胀缓解和排气、排便时间均显著短于对照组,2组比较差异均有统计学意义(P<0.05)。治疗后2组患者的血清TNF-α、IL-6、IL-8水平均显著降低,与治疗前比较差异均有统计学意义(P<0.05或P<0.01);且治疗后观察组患者的血清TNF-α、IL-6、IL-8水平显著低于对照组,2组比较差异有统计学意义(P<0.05)。【结论】大承气汤联合血必净注射液治疗早期重症急性胰腺炎患者能显著提高疗效,这可能与其能拮抗血清TNF-α、IL-6、IL-8等炎症因子的释放有关。
Objective To observe the therapeutic effect of Potent Chengqi Decoction （PCD） combined with Xuebijing Injection （XI） for the treatment of early severe acute pancreatitis （SAP）. Methods Seventy-one early SAP patients were randomized into treatment group （N=36）and control group （N=35）. Both groups were given conventional western medical treatment, and the treatment group was given intranasal feeding or enema of PCD combined with intravenous injection of XI additionally. The therapeutic effect was evaluated in both groups, the time for the recovery of gastrointestinal function was examined, and the changes of serum tumor necrosis factor （TNF- α）, interleukin-6 （IL- 6） and IL-8 were observed. Results The therapeutic effect in the treatment group was superior to that in the control group, the time for relieving abdominal pain and distention, and time for exsufflation and defecation were shorter in the treatment group （P〈0.05） . After treatment, serum TNF-α, IL- 6 and IL- 8 levels were decreased in both groups （P〈0.05 or P〈0.01 compared with those before treatment）, and were much lower in the treatment group than those in the control group after treatment （P〈0.05）. Conclusion PCD combined with XI is effective for the treatment of early SAP, and its therapeutic mechanism may be related with inhibiting the release of inflammatory factors of TNF- α, IL- 6 and IL- 8.
