摘要
目的探讨骨科Ⅰ类切口手术患者预防性应用抗菌药物的临床效果。方法将骨科收治的Ⅰ类切口择期手术患者300例,随机分为方案组149例及对照组151例。对照组采用盲法(不进行干预,按医生习惯)给予抗菌药物;方案组按照制定的方案预防性应用抗菌药物。比较两组体温变化、切口愈合、切口感染情况及主要经济学指标。结果两组患者切口均愈合,且未出现切口感染、呼吸道感染及泌尿系统感染患者,术后体温异常发生率差异无统计学意义。方案组77.2%的患者仅在术前使用抗菌药物1次,22.8%的患者术前及术后均使用抗菌药物,所有患者术前及术后抗菌药物使用时间均为24h以内,而对照组患者术后抗菌药物平均使用(5.43±2.72)d。方案组平均住院天数、住院总费用、药品费用及抗菌药物费用均显著低于对照组。结论术前预防性应用抗菌药物并在24h内停药可显著缩短住院时间,降低住院相关的各项费用,促进骨科Ⅰ类切口手术围手术期预防用抗菌药物规范化及合理化,值得推广。
Objective To explore the clinical effect of prophylactic application of antibacterials in orthopedics patients with type Ⅰ incision operation. Methods Three hundred patients with type Ⅰ incision and selective operation were randomly divided into schema group (n= 149) and control group (n=151). Control group was adopted a blinded method to give antibacterials (without intervention according to the doctor's habit),while antibacterials were used preventively according to plan in schema group. Abnormal body temperature, wound healing, wound infection and main cconomic indicators were compared in 2 groups. Results The incisions of all patients in 2 groups got healing, anti there were no patients with incision, respiratory tract and urinary tract infection. Significant difference was not presented regarding the incidence of abnormal body temperature between 2 groups. 77.2% of patients in schema group received antibacterials only once before operation, and 22.8 %received antibacterials before and after operation. The application time of antibacterials in all patients before and after operation were all within 24 hours, while that in control group after operation was (5.43±2.72) days. The average hospital stays,hospitalization costs, drug costs and antibacterials costs in schema group were all obviously lower than those in control group. Conclusion Prophylactic application of antibacterials within 24 hours can significantly shorten hospital stays, reduce hospitalization-related costs, and promote the standardization and rationalization of prophylactic application of antibacterials in our hospital orthopedics patients with type Ⅰ incision operation,being worthy of promotion.
