摘要
目的比较抗菌药物临床应用专项整治活动前后抗菌药物应用变化。方法采取回顾性调查方法,以抗菌药物应用指标、Ⅰ类切口围手术期抗菌药物应用、冠脉介入诊断抗菌药物预防应用及细菌学检查等为观察指标,比较专项整治前后抗菌药物临床应用情况。结果整治活动6个月后,住院、门诊患者抗菌药物使用率和住院患者使用强度分别由63.37%、23.67%、65.10DDDs/(100人.d)下降至44.99%和14.90%、53.89DDDs/(100人.d);Ⅰ类切口围手术期预防性使用抗菌药物的品种选择和用药时机合理率及用药时间符合率均明显改善,使用率和联合用药率显著下降;冠脉介入诊断不应用抗菌药物;微生物送检率提高。结论专项整治活动能显著改善抗菌药物临床应用的合理性,但仍未完全达到卫生部的要求,需加大力度进一步整治。
OBJECTIVE To compare the clinical application of antibiotics before and after the special rectification activities.METHODS By means of retrospective investigation,the indicators including the use of antibiotics,the use of antibiotics during the perioperative period of type Ⅰ incision surgery,prophylactic use of antibiotics for coronary artery intervention diagnosis and the detection of bacteriology were observed,the clinical application of antibiotics was compared before and after the special rectification activities.RESULTS After the special rectification activities for 6 months,the utilization rates of antibiotics of the hospitalized patients and the outpatients decreased respectively from 63.37% and 23.67% to 44.99% and 14.90%;the DDDs of the hospitalized patients decreased from 65.10DDDs/100 patient-day to 53.89DDDs/100 patient-day;the use of antibiotics during the perioperative period of type Ⅰ incision surgery tended to be reasonable,the rationality of the medication timing and the medication accordance were significantly improved,the utilization rate and the combination use rate were both decreased significantly;the rational application of antibiotics during perioperative period and coronary artery intervention diagnosis were improved too;the submission rate of microorganism was elevated.CONCLUSION The special rectification activities can significantly improve the rationality of clinical application of antibiotics which remains not completely meeting the standard of the ministry of health so that it is necessary to further rectify.
