2型糖尿病下肢血管病变患者血浆Omentin-1水平及与IL-6、hs-CRP的相关性 被引量:9

PLASMA OMENTIN-1 LEVELS IN PATIENTS WITH TYPE 2 DIABETES ACCOMPANIED WITH LOWER EXTREMITY ARTERIAL DISEASE AND RELATIONSHIP BETWEEN OMENTIN-1,IL-6 AND CRP LEVELS

摘要 目的探讨2型糖尿病(type 2 diabetes mellitus,T2MD)下肢血管病变患者血浆网膜素1(Omentin-1)水平及与白细胞介素6(interleukin-6,IL-6)、超敏C反应蛋白(high-sensitivity C-reactive protein,hs-CRP)的相关性,阐明Omentin-1在T2DM大血管病变发生发展中的作用机制。方法 T2DM患者90例,根据踝肱指数(ankle brachialindex,ABI)分为T2DM未并发下肢血管病变(non-peripheral arterial disease,NPAD)组和T2DM并发下肢血管病变(peripheral arterial disease,PAD)组,同时选择我院健康体检者35例作为正常对照(normal control,NC)组,采用酶联免疫吸附测定法测定血浆Omentin-1、IL-6和hs-CRP水平,并分析血浆Omentin-1与IL-6、hs-CRP及糖脂代谢指标的关系。结果与NC组比较,PAD组、NPAD组空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting plasmainsulin,Fins)、胰岛素抵抗指数(homeostasis model assessment-insulin resistance index,HOMA-IR)、三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、IL-6和hs-CRP明显增高,而血浆Omentin-1水平明显降低(P<0.05或<0.01);与NPAD组比较,PAD组Fins、HOMA-IR、IL-6和hs-CRP明显增高,而血浆Omentin-1水平明显降低(P<0.05或<0.01)。T2DM患者Omentin-1与体质量指数(body mass index,BMI)、腰臀比(waist hip ratio,WHR)、FBG、TG、IL-6、hs-CRP、HOMA-IR和Fins呈明显负相关(P<0.05或<0.01)。hs-CRP和HOMA-IR是影响PAD组患者血浆Omentin-1水平的独立相关因素。结论 T2DM患者尤其是伴有下肢血管病变患者血浆Omentin-1水平与炎性反应和胰岛素抵抗密切相关,此为早期诊断糖尿病下肢血管病变及预测其预后提供了理论依据。 Objective To investigate plasma Omentin-1 levels in patients with type 2 diabetes mellitus (T2MD) accompanied with lower extremity arterial disease and relationship between interleukin- 6 (IL-6),high-sensitivity C-reactive protein (hs-CRP) and Omentin-1 levels and explore the role and mechanisms of change of Omentin-1 in patients with diabetic macrovascular disease. Methods Ninety patients with T2DM were divided into peripheral arterial disease (PAD) group and non-peripheral arterial disease (NPAD) group according to the ankle brachial index (ABI), and meanwhile 35 patients of health examination were selected as normal control (NC) group. Fasting plasma Omentin-1 level was measured by enzyme-linked immunosorbent assay (ELISA). The relationship between change of Omentin-1, IL-6,hs-CRP and other glucolipid metabolism parameters were also analyzed. Results Compared with those of control group, fasting blood glucose ( FBG), fasting plasma insulin ( Fins), homeostasis model assessment- insulin resistance index ( HOMA-IR ), triglyeride ( TG ), total cholesterol ( TC ), IL-6 and hs-CRP in patients with PAD and NPAD groups were significantly increased, but plasma Omentin-1 levels were significantly decreased(P 〈0.05 or 〈0.01 ). Compared with those of NPAD group,Fins,HOMA-IR,IL-6 and hs-CRP, in patients with PAD group were significantly increased, but plasma Omentin-1 levels were significantly decreased (P 〈 0.05 or 〈 0.01 ). Omentin-1 was correlated negatively with body mass index (BMI) ,waist hip ratio (WHR), FBG,TG, IL-6, hs-CRP, HOMA-IR and Fins in patients with T2DM (P 〈 0.05 or 〈 0.0! ). Hs-CRP and HOMA-IR were independent related factors of Omentin-1 in patients with PAD. Conclusion Plasnm Omentin-1 levels are well correlated with inflammatory reaction and insulin resistance in T2DM patients or accompanied with lower extremity arterial disease, which can provide theoretical basis for early diagnosis of diabetic lower extremity arterial disease and forecast its prognosis
