血浆富含半胱氨酸的酸性蛋白酶与2型糖尿病下肢大血管病变的关系

摘要 目的探讨2型糖尿病(T2DM)患者血浆富含半胱氨酸的酸性蛋白酶(SPARC)与下肢大血管病变的关系,阐明SPARC在2型糖尿病下肢大血管病变发生、发展中的作用机制。方法 T2DM合并下肢血管病变患者32例作为病变组,无下肢血管病变的T2DM患者29例作为DM组,同期选择我院健康体检25例作为对照组,采用酶联免疫法测定血浆SPARC水平,并分析血浆SPARC水平与体重指数(BMI)、腰臀比(WHR)、肿瘤坏死因子(TNF)-α、踝肱指数(ABI)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)等的关系。结果病变组血浆SPARC水平均明显高于DM组和对照组([19.35±5.62)μg/L比(17.48±4.71)μg/L,(12.19±3.80)μg/L,均P<0.01],且DM组血浆SPARC水平明显高于对照组([17.48±4.71)μg/L比(12.19±3.80)μg/L,P<0.01]。T2DM患者SPARC与BMI、TNF-α、TG、FBG、FINS、HOMA-IR呈明显正相关(P<0.01或P<0.05),与ABI成明显负相关(P<0.01)。BMI、HOMA-IR和ABI为影响T2DM患者SPARC的独立相关因素。结论血浆SPARC水平与T2DM患者糖脂代谢紊乱、炎症反应及胰岛素抵抗密切相关,可能在T2DM下肢大血管病变的发生发展中起着重要作用。 Objective To investigate the relationship of secreted protein acidic and rich in cysteine (SPARC) with lower extremity arterial disease in type 2 diabetic patients T2DM) and explore on role and mechanisms of SPARC in lower extremity arterial disease of type 2 diabetes mellitus. Methods Thirty-two T2DM patients complicated by lower extremity arterial disease (disease group),29 T2DM patients without lower extremity arterial disease (DM group), and meanwhile choose 25 patients of health examination as control group. SPARC levels were assayed by ELISA. The relationship between SPARC levels, body mass index (BMI),waist hip ratio (WHR),tumor necrosis factor alpha (TNF-α),ankle brachial index (ABI),fasting plasma insulin (FINS) and HOMA-insulin resistance index (HOMA-IR),other metabolic parameters was also analyzed. Results SPARC levels in patients with disease group were significantly increased compared with DM group and control group [(19.35±5.62)μg/L vs(17.48±4.71)μg/L,(12.19±3.80)μg/L,P0.01]; SPARC levels were higher in DM group than that control group[(17.48±4.71)μg/L vs(12.19±3.80)μg/L,P0.01]. SPARC was correlated positively with BMI,TNF-α,triglyeride (TG),fasting blood glucose (FBG),FINS and HOMA-IR (P0.01 or P0.05),and negatively with ABI (P0.01). BMI,HOMA-IR and ABI were independent related factors in influencing ABI in T2DM patients. Conclusion The changes of plasma SPARC levels are associated with glucose and lipid metabolism disorder,inflammatory reaction and insulin resistance (IR),and it may contribute,in part to the pathogenesis of T2DM patients complicated by lower extremity arterial disease.
