摘要
目的 了解梗阻性黄疸 (Obstructivejaundice,OJ)形成过程中内皮素 (Endothelin ,ET)在血浆及肝组织中的水平及其在OJ肝损伤中的作用。方法 以大鼠胆总管结扎 (Bileductligation ,BDL)为模型 ,采用放射免疫法 (Radioimmunas say,RIA)测定了BDL后 3、6、9、1 2、1 5d时血浆及肝组织中ET的含量 ,同时观测血清谷丙氨酸转氨酶 (ALT)含量变化 ,并与肝组织病理改变进行对照分析。结果 BDL后血清总胆红素 (Totalbilirubin ,TBi)迅速升高 ,明显高于正常组和假手术组 (P <0 0 1 ) ;BDL组血浆及肝组织中ET水平明显高于对照组 (P <0 0 1 ) ,BDL各时相组之间有显著性差异 (F =1 8 4 1 ,P <0 0 1和F =4 0 61 ,P <0 0 1 ) ,随梗阻时间延长呈进行性含量升高 ,1 5dET值最高 ,分别为 ( 2 94 98± 3 5 95)ng/L和 ( 2 0 1 86± 2 4 4 5)ng/ 1 0 0g;二者与血清ALT均呈正相关 (r=0 84 41 ,P <0 0 1和r=0 93 0 3 ,P <0 0 1 ) ;BDL +ET -Ab组血浆ET含量较BDL组降低 (P <0 0 5) ,且ALT水平也下降 (P <0 0 1 )。结论 提示在OJ形成中血及肝组织中ET含量上升 ,并随梗阻时间延长而加重 ;ET -Ab有降低ET含量及减轻肝损伤的作用 ,ET可能是参与OJ肝脏损伤的重要因素之一 ;
Objective To investigate endothelin levels and evaluate the role of endothelin in liver damage during the formation of experimental rat obstructive jaundice.Methods The concentrations of endothelin in plasma and hepatic tissues were determined on 3,6,9,12 and 15 hours after bile duct ligation(BDL) by radioimmunoassay.The histopathological changes of hepatic tissues were examined.Results The resulte showed that serum total bilirubin was elevated rapidly after BDL and increased markedly,compared than normal control and sham-operated rats (P<0 01 and P<0 01,respectively).The concentration of endothelin after BDL increased significantly, compared with normal control and sham operation (P<0 01 and P<0.01) were elevated as the time capsed.The peak values appeared on 15 d after BDL (294.98<35.95 ng/L and 201.86±24.45 ng/100 g,respectively).Endothelin levels in plasma-hepatic tissues and serum ALT were well correlated (r=0.8441,P<0.01,and r=0.9303,P<0.01,respectively).Plasma ET and ALT levels were markedly decreased by using ET-Ab.Histological examination showed that hepatic cells were damaged and were consistent with concentration of endothelin progressively with prolongation of BDL.There was lots of cholangioloe hyperplasia and the changing rules were quite similar to levels.Conclusions It is suggested that endothelin in plasma and hepatic tissues increase significantly in BDL,and ET-Ab effectively alleviates the damage of liver,and may be an important factor leading to damage of liver in obstructive jaundice.
