摘要
目的评价改良尺骨鹰嘴双向牵引治疗儿童肱骨髁上骨折的临床效果和安全性,并观察即刻功能锻炼对骨折患儿上肢功能恢复的影响。方法对2009年1月～2012年6月在长沙市中医医院骨科接受治疗的肱骨髁上骨折136例患儿采取两种不同的方法进行治疗。其中,68例(观察组)采取改良尺骨鹰嘴双向牵引配合即刻功能锻炼治疗,68例(对照组)采用四合一夹板外固定治疗,比较分析两组的治疗效果和并发症。结果治疗后所有患儿均得到随访,观察组患儿骨折愈合优良率优于对照组,差异有统计学意义(χ2=5.12,P<0.05);观察组患儿的住院时间、治疗费用等均低于对照组,差异有统计学意义(t=4.31、12.93,P<0.05)。结论改良尺骨鹰嘴双向牵引治疗儿童肱骨髁上骨折具有良好的疗效,牵引治疗后对患儿采取及时正确的健康指导性即刻功能锻炼,能够显著的降低并发症的发生率,提高患儿上肢功能的恢复速度,是治疗儿童肱骨髁上骨折的理想方法,具有临床推广意义。
Objective To evaluate the improved olecranon bidirectional traction tor the treatment ot chmcal eltlcacy and safety of children supraeondylar fractures, and to observe the immediate functional exercise recovery of fractures in children with upper extremity function. Methods The period from January 2009 to June 2012 in Department of Orthopedic, Changsha Chinese Medicine Hospital for treatment of humeral fractures in children with 136 cases of children taken two different approaches for treatment. Of these, 68 patients （observation group） were treated in the improved olecranon bi-directional traction with the immediate exercise therapy, 68 patients （control group） were treated in quadruple splint external fixation, treatment effect and complications of the two groups were compared. Results After treatment, all patients were followed up, the observation group were excellent rate of fracture healing than the control group, had a significant difference （X^2 = 5.12, P〈 0.05）; time of the observation group were hospitalized, were lower than the cost of treatment control group, the difference was statistically significant （t = 4.31, 12.93, P 〈 0.05）. Conclusion The improved oleeranon bidirectional traction treatment of children＇s supracondylar femur fracture and having good ef- fect, to take timely and correct health guidance instantly functional exercise improved olecranon traction treatment of children, can significantly reduce the incidence of complications, improve the speed of recovery of upper limb function, it is the ideal method for the treatment of children supracondylar femur fractures with clinical significance.
