摘要
目的:探讨血清胱抑素C(CysC,反映肾小球滤过率)相关指标预测老年高血压患者肾功能状况。方法:采用免疫比浊法对54例老年高血压病患者,进行血清CysC及其相关指标检测,根据CysC是否升高分为CysC正常高血压组(20例),CysC异常高血压组(34例),并与健康老年人(健康对照组,43例)进行比较,同时分析Cy-sC与各指标之间的相关性。结果:与健康对照组和CysC正常高血压组比较,CysC异常高血压组同型半胱氨酸[Hcy:(10.092±0.916)μmol/L、(6.980±0.582)μmol/L比(17.668±1.787)μmol/L]、尿β2-微球蛋白[尿β2-MG:(2.23±1.07)ng/ml、(2.347±0.078)ng/ml比(4.693±0.405)ng/ml]水平均明显升高(P均<0.05);Pearson相关分析发现,老年高血压患者血清CysC水平升高和尿β2-MG(r=0.814,P<0.01)、Hcy(r=0.838,P<0.01)、年龄(r=0.897,P<0.01)呈正相关;Logistic多元回归分析也证实CysC与后三者存在线性关系,回归方程:CysC=-0.958+0.29β2-MG+0.10Hcy+0.24年龄(R2=0.921,df=53,P=0.000)。结论:在老年高血压病人中,血清同型半胱氨酸和尿β2-微球蛋白的异常,可能预示有肾功能损害,并且随着年龄的增长,这种损害程度会进一步加重。
Objective： To investigate indexes related with serum cystatin C （CysC, reflecting glomerular filtrationrate） predicting renal function status in aged patients with hypertension. Methods： Serum CysC and its relative inde- xes homocysteine （Hcy）, urinary β2 microglobulin （urinary β2-MG） were measured by turbidimetric immunoassay in 54 aged patients with hypertension. According to CysC level increased or not, they were divided into normal CysC hypertension group （n = 20） and abnormal CysC hypertension group （n = 34）, and compared with healthy aged peo- ple （healthy control group, n = 43）. Meanwhile, correlation among its relative indexes and CysC were also ana- lyzed. Results： Compared with healthy control group and normal CysC hypertension group, there were significant increase in levels of homocysteine [Hcy： （10. 092 ± 0. 916） μmol/L, （6. 980 ±0. 582） mol/L vs. （17. 668± 1. 787） mol/L] and urinary β2 microglobulin [urinary β2-MG： （2.23 ± 1.07） ng/ml, （2. 347± 0. 078） ng/ml vs. （4. 693 ±0. 405） ng/ml] in abnormal CysC hypertension group, P〈0.05 all; Pearson correlation analysis found that in- creased serum CysC level was positively correlated with urinary β2-MG （r = 0. 814, P〈0.01）, Hcy （r = 0. 838, P d0. 01） and age （ r = 0. 897, P〈0.01） in aged patients with hypertension; Logistic multiple regression analysis also proved that there＇s a linear relationship among β2-MG, Hcy, age and CysC, and regression equation was CysC = - 0. 958 ＋ 0.29 β2-MG ＋ 0.10 Hcy ＋ 0.24 age （R2 = 0. 921, df = 53, P = 0. 000）. Conclusion= Among aged patients with hypertension, abnormal serum homocysteine and urinary β2 microglobulin levels may indicate occurrence of re- nal function damage. As age increases, the damage degree aggravates.
