摘要
目的研究长期高血糖对2型糖尿病患者听觉传出神经功能的影响.方法分别对27例糖尿病患者(糖尿病组)及28例健康者(对照组)行纯音听阈、声导抗、畸变产物耳声发射(DPOAE)检测,并通过在其对侧耳中加入60dB SPL 的宽带噪声,进行 DPOAE 对侧抑制效应测试以评估其传出神经功能.结果两组对象纯音听阈在各个频率上的差异均无统计学意义(均 P 〉0.05).糖尿病组患者 DPOAE 在中低频率段(750~2000Hz)处明显低于对照组(均 P〈0.05).进行对侧抑制后,糖尿病组患者DPOAE 在750、1000、3000Hz 处明显降低(均 P〈0.05),对照组 DPOAE 在中低频率段(750~2000Hz)明显降低(均 P〈0.05).糖尿病组患者在中低频段(750~2000Hz)的 DPOAE 对侧抑制效应明显低于对照组,差异有统计学意义(P〈0.05).结论糖尿病患者听觉传出神经的功能受到损害,这有可能会降低其在噪声环境下对言语的识别能力.
Objective To investigate the auditory efferent system function in type 2 diabetic(T2D) patients. Methods The pure tone audiometry, acoustic immittance and distortion product otoacoustic emissions (DPOAE) tests were performed in 27 T2D patients and 28 healthy controls. The efferent system functions were evaluated by contralateral suppression of DPOAE through applying 60dB SPL broadband noise to contralateral ear. Results There were no significant differences of pure tone thresholds between the two groups. The DPOAE in low frequencies and mid-frequencies (750~2 000Hz)in diabetic patients were signifi-cantly lower than that in controls (P〈0.05). Under the condition of contralateral suppressions, the DPOAE in diabetic patients was decreased at 750Hz, 1 000Hz and 3000Hz and the DPOAE in controls also went down sharply in low frequencies and mid-frequencies(750~2 000Hz)(P〈0.05). The DPOAE contralateral suppression effect at low frequencies and mid-frequencies (750~2 000 Hz) in diabetic patients was significantly lower than that in controls (P〈0.05). Conclusion The auditory efferent sys-tem functions in diabetic patients are damaged, leading to a negative effect on their speech perception in noisy environment.
