社区慢性病管理对青光眼病情影响的初步研究

摘要
青光眼是最主要的致盲眼病…，若能早期发现、终身定期随访和监测及有效控制眼压，大多数因青光眼而致盲是可避免的。我中心对部分慢性青光眼患者进行社区为基础的慢性病管理，开展科普教育，定期随访眼压，对眼压异常者实现转诊，由上级医院进行眼压控制。研究目的在于评估该慢性病管理模式对青光眼患者关于青光眼知识的认知、视功能维持的作用。
