摘要
目的探讨血液透析滤过(hemodiafiltration,HDF)对尿毒症患者瘦素及营养状态的影响。方法将40例尿毒症维持性透析患者随机分为2组,每组20例,实验组应用血液透析(HD)及血液透析滤过(HDF)交替治疗,对照组应用HD治疗,所有患者每周透析3次,每次4小时,并观察治疗前后6个月患者瘦素(Leptin)、血浆白蛋白(Alb)、转铁蛋白(TF)、血红蛋白(Hb)、体质量指数(BMI)、体力、食欲及一般情况等项目,并比较分析。结果经治疗6个月后,实验组与对照组的观察指标Leptin、Alb、TF、Hb、BMI及体力、食欲、一般情况等比较差异有统计学意义,分别为Leptin(6.43±2.75)μg/L vs(9.65±3.23)μg/L、Alb(42.39±4.52)g/L vs(37.85±4.83)g/L、TF(3.74±0.36)g/L vs(3.23±0.28)g/L、Hb(106.39±22.78)g/L vs(101.56±23.70)g/L、BMI(23.31±5.21)vs(20.03±5.11)(P<0.05或<0.01)。结论 HDF能明显改善透析患者的营养状态,是值得推广的透析方式。
Objective The study was to evaluate the effects of hemodiafihration(HDF) on leptin and nutrition status in patients with uremia. Methods Forty cases of maintenance hemodialysis patients were randomly divided into two groups:hemoperfusion(HD) group(20 cases) and HDF group(20 cases). HD group used HD treatment, HDF group used HD and HDF alternative treatment. All patients were treated four hours a time and three times a week. I.eptin, plasma albumin ( Alb ), transferionprotein (TF), hemoglobin (HB), body mass index ( BMI), physical strength, appetite and general conditions were examined and compared before treatment and after treatment for six months. Results After treatment for six months,leptin,Alb,TF,ttb,BMI,physical strength,appetite and general conditions in HDF group showed more significant improvement as compared with those in HD group, leptin(6.43 ±2.75) μg/L vs (9.65±3.23) μg/L,Alb(42. 39±4. 52) g/L vs (37.85±4.83) g/L,TF(3.74±0.36) g/L vs (3.23±0.28) g/L,Hb (106.39±22.78) g/L vs (101.56±23.70) g/L,BMI (23.31±5.21) vs (20.03±5.11)(P 〈0.05 or 〈0.01), respectively. Conclusion HDF is a better dialysis method for improving the nutrition status in renal dialysis patients with uremia.
