摘要
目的评价中国(未包括香港、澳门特别行政区和台湾地区)2011年麻疹监测系统(MeaslesSurveillance System,MSS)的运转状况,为加强麻疹监测、消除麻疹提供参考。方法 分析31个省(自治区、直辖市,下同)2011年通过MSS上报的疑似麻疹病例个案数据库,评价MSS的主要运转指标。结果全国2011年共报告疑似麻疹34642例,其中9943例为麻疹,排除麻疹24692例,另有7例疑似麻疹分类待定。全国2011年排除麻疹病例报告发病率为1.84/10万,有14个省(占45.2%)>2/10万,42.0%的县(区、市、旗)>1/10万。34642例疑似麻疹病例中,93.30%在报告后48h内进行个案调查(28个省>80%);散发病例血标本采集率90.37%(28个省>80%)。全国2011年共采集疑似麻疹病例血标本33362份,其中93.85%麻疹免疫球蛋白M检测结果报告日期距采血日期≤7d(28个省>80%)。2011年共报告62起麻疹爆发疫情,其中98.38%为经实验室确诊,47起(75.81%)采集了病原学标本。结论中国MSS的特异性和及时性监测指标达到了卫生部《全国麻疹监测方案》要求,但各省MSS运转质量不均衡,需进一步提高疑似麻疹病例监测敏感性,以适应消除麻疹需要。
Objective To evaluate the running status of measles surveillance system (MSS)of China(Hong Kong special administrative region, Macao special administrative region, Taiwan Sheng were excluded)in 2011. Methods The investigation data of suspected measles cases reported by 31 provinces (municipals, autonomous regions)through MSS in 2011 were reviewed. Performance indicators of the MSS were analyzed. Results Totally 34,642 suspected measles cases were reported through MSS in 2011, of which 9,943 were classified as laboratory or clinically confirmed measles cases, 24,692 were discarded measles cases, and 7 were unclassified. In 2011, reported incidence of discarded measles case was 1.84 per 100,000 population, and 42.0%of all counties 〉 1 per 100,000 population.Proportion of suspected measles cases investigated within 48 hours of report was 93.30% (28 provinces 〉80%), proportion of suspected measles cases with adequate serum specimen was 90.37% (28 provinces 〉80%), and percentage of serum specimen with laboratory results reported within 7 days of collection was 93.85% (28 provinces 〉80%). Among the 62 measles outbreaks, 98.38 %were laboratory-confirmed whereas 75.81%with specimen collected for virus detection. Conclusions Our finding demonstrated that the MSS is effective in measles surveillance, with improved specificity by means of case investigation and laboratory testing. However, each province should increase thesensitivity of monitoring so as to reach the goal of measles elimination.
