摘要 目的 探讨卵巢癌患者的细胞免疫状态以及 IL- 2 ,血清游离白细胞介素 - 2受体 (s IL- 2 R) ,外周血单个核细胞(PBMC)膜结合型白细胞介素 - 2受体 (m IL- 2 R)与临床的关系 .方法  3H- Td R参入法测 31例卵巢癌患者 IL- 2产生水平 ,EL ISA法检测血清 s IL - 2 R水平 ,APAAP免疫组化检测外周血单个核细胞膜结合型 IL - 2 R表达细胞 (CD2 5 + 细胞 ) .结果  31例卵巢恶性肿瘤患者术前及术后 7d,IL- 2产生能力降低 ,s IL- 2 R水平增高 ,与术后第 6疗程化疗结束后 4wk相比有显著差异 (P<0 .0 1) ;术前临床 ~ 期患者 IL- 2活性低于 ~ 期患者 ,s IL- 2 R水平明显高于 ~ 期患者 (P<0 .0 5 ) ;CD2 5 + 细胞均为个别阳性 ,各组间无差异 ;术后复发的 5例患者血清 s IL - 2 R持续增高 .结论 对 IL - 2及 s IL - 2 R的定期检测 ,有助于观察卵巢恶性肿瘤患者病情变化 。 AIM To unveil the immunological state of patients with ovarian cancer and the relationship between IL 2, IL 2R and the clinical manifestation. METHODS 31 patients with ovarian cancer were examined for IL 2 production by 3H TdR incorporation assay, sIL 2R by ELISA and expression of CD25 by APAAP immunohistochemistry. RESULTS Compared with those of patients six months after operation who had received regular chemotherapy , IL 2 production by peripheral blood mononuclear cells (PBMC) was significantly lower and the serum sIL 2R levels were significantly increased among patients 7 days before and after operation ( P <0.01). In comparison with those of patients with ovarian cancer of stage Ⅰ~Ⅱ, PBMC IL 2 production was significantly decreased and serum sIL 2R levels were significantly higher in the advanced cases (stage Ⅲ~Ⅳ) before operation ( P <0.05). These differences disappeared after patients received operative therapy. Very few CD25 + cells were observed in all patients. There was a persistently high serum level of sIL 2R in the 5 relapsed patients. CONCLUSION The progress of cancer may be associated with the IL 2 production and serum level of the sIL 2R. The result indicates that IL 2 and sIL 2R can be used to assess the progress of cancer and are helpful markers in predicting the prognosis.
