生物反馈联合盆底肌训练对阴道分娩会阴重度裂伤后排便功能的疗效 被引量:14

摘要 目的:探讨生物反馈联合盆底肌训练对促进阴道分娩会阴重度裂伤修补术后排便功能的疗效。方法:阴道分娩会阴重度裂伤患者30例,随机分为观察组和对照组各15例,均给予会阴重度裂伤术后常规康复护理,观察组于产后第7天加用生物反馈联合盆底肌训练康复治疗。训练前后记录患者肛门失禁临床评分和肛门直肠侧压指标,并评估临床疗效。结果:训练4个疗程后,2组肛门失禁临床评分均较训练前明显降低,且观察组更低于对照组(均P<0.05);2组肛管静止压、肛管最大收缩压、肛管最大收缩压持续时间均较治疗前明显提高,且观察组提高更显著(均P<0.05),2组直肠感觉阈较治疗前明显降低,且观察组降低更显著(均P<0.05);临床疗效比较,观察组总有效率明显高于对照组(P<0.05)。结论:生物反馈联合盆底肌训练有利于产后会阴重度裂伤患者排便功能恢复,是安全、有效、价廉的方法。 Objective:To study the effect of biofeedback combined with pelvic floor muscle training on recovery of defecation after serious laceration ofperineum after vaginal delivery. Methods:Thirty cases of serious perineum lacer- ation after vaginal delivery were divided into two groups randomly and equally. Both groups received routine therapy for serious vaginal laceration, and observation group was subjected to biofeedback therapy combined with pelvic floor muscle training from the 7th day of postpartum. Clinical scores of the anal incontinence, the index of digital examina- tion of rectum, and clinical effects were recorded. Results:After 4 courses of treatment, the clinical scores of the anal incontinence in both groups were decreased,and more significantly in observation group than in control group(all P d0.05). The indexes of digital examination of the rectum in both groups were improved, and more significantly in observation group than in control group(all P^0.05). The effective rate in observation group was obviously higher than in control group(P^0.05). Conclusion:The treatment of biofeedback combined with pelvic floor muscle train- ing is beneficial for defecation function recovery after reparation of the serious laceration perineum following vaginal delivery. It is a secure, effective and inexpensive method to patients.
