卵泡输出率与体外受精-胚胎移植中卵巢反应性的研究 被引量:6

Study on the follicular output rate and ovarian response in the IVF-ET patients

摘要 目的探讨卵泡输出率(FORT)对实施体外受精-胚胎移植(IVF-ET)患者卵巢反应性及妊娠结局的预测效力。方法按FORT值将200个IVF-ET患者分为高、低2组,回顾性分析2组间控制性超排卵数据及实验室指标。结果高FORT值组降调后窦卵泡数少于低FORT值组[(10.9±3.5)与(11.1±6.2)个,P<0.001],而HCG日E2水平明显高于低FORT值组[(2 701.2±102)与(2 183±86)pg/ml,P<0.001)],但2组间基础FSH及E2水平、促性腺激素总量及排卵前卵泡数差异无统计学意义(P>0.05)。高FORT值组获卵数(11.8±0.6)与(8.6±0.4)个;MⅡ卵数(10.0±0.6)与(7.4±0.4)个;可移植胚胎数(7.4±0.4)与(5.7±0.3)个;胚胎种植率37.7%与23.3%;临床妊娠率62.8%与33.8%,均高于低FORT值组(P<0.05)。结论 FORT可有效地预测IVF-ET过程中卵巢的反应性,高FORT值患者将具有更好的妊娠结局。 Objective To evaluate if the follicular output rate (FORT) is an effective parameter for predicting the ovarian response in IVF - ET. Methods 200 IVF - ET patients were divided into two groups according to their FORT. The parameters of the controlled ovarian hyperstimulation and IVF - ET between these two groups were retrospectively analyzed. Results The antral follicle count of the high FORT group after pituitary desensitization was less than the low FORT group ( 10.9 ± 3.5 vs. 11.1± 6.2,P 〈0. 001 ). However, the concentration of serum E2 on the day of HCG was significantly higher in the high FORT group (2701.2 ± 102 vs. 2183± 86, P 〈0.1301 ). There were no differences about the basal FSH, basal E2, the total gonadotropin dosage and the preovulatory follicle count ( all P 〉0. 05 ) between two groups. The oocytes retrieved number ( 11.8±0. 6 vs. 8.6 ±0. 4, P 〈 0. 001 ), M 11 oocytes number ( 10.0 ± 0.6 vs. 7.4 ±0. 4, P 〈 0. 001 ), embryos obtained number (7.4 ±0. 4 vs. 5.7 vs 0.3, P 〈 0. 002), the embryo implantation rate (37.7% vs. 23.3% , P 〈0. 004) and clinical pregnancy rate (62.8% vs. 33.8% , P 〈 0. 004) were remarkably higher in the high FORT group. Conclusion The study suggests that FORT may be a qualitative reflector of ovarian follicular competence and ovarian response. IVF -ET candidates with moderately high FORT may have more chances for pregnancy.
