Changes of circulating endothelial progenitor cells in patients with H-type hypertension

摘要 目的 观察H型高血压患者循环内皮祖细胞(EPC)的改变.方法 选取H型高血压患者30例(观察组)及非H型高血压患者30例(对照组).采用自动荧光偏振免疫分析实验检测血浆同型半胱氨酸(Hcy)水平,流式细胞仪检测外周血EPC数量,改良Boyden小室法观察EPC的迁移能力,ELISA法测定细胞培养上清液细胞因子SDF-1水平.结果 观察组患者血浆Hcy水平、循环EPC数量、EPC迁移能力、EPC培养上清液SDF-1表达水平分别为(14.89±3.37)μmol/L、(100.43±12.36)个/ml、(48.27±14.59)个/mm^2、(520.87±145.03)pg/106,对照组分别为(9.39±2.50)μmol/L、(124.03±12.87)个/ml、(61.57±12.86)个/mm^2、(673.77±168.32)pg/106.两组患者血浆Hcy水平、循环EPC数量、EPC迁移能力及EPC培养上清液SDF-1表达水平的差异均有统计学意义(均P<0.01).观察组患者循环EPC数量与血浆Hcy水平呈明显的反向线性关系(P<0.01).结论 H型高血压患者循环EPC数量、迁移和分泌能力较非H型高血压患者进一步下降,并且该变化可能与血浆Hcy水平升高有关. Objective To investigate the changes of numbers and activities of circulating endothelial progenitor cells (EPCs) in essential hypertensive patients with hyperhomocysteinemia (HHcy). Methods Human EPCs were isolated and cultured from blood samples of H-type essential hypertensive patients (n=30) and matched non-H-type hypertensive patients (n=30). Numbers of circulating EPCs were detected in peripheral blood by flow cytometry. EPCs were expanded from human blood in vitro and identified by Dil-acLDL uptake and lectin staining by direct fluorescent staining under a laser scanning confocal microscope. EPC migration activities were determined by modified Boyden chamber assay. Concentration of SDF-1 cytokine was determined in supernatant of cell culture. Results A significant decrease was observed in numbers of circulating EPC of essential hypertensive patients with HHcy compared with controls (100.43 + 12.36 cells/ml vs. 124.03 ± 12.87 cells/ml blood, P〈 0.01). Moreover, there was a negative linear correlation (r=-0.731, P〈0.001) between numbers of EPCs and the plasma homo- cysteine concentration in patients. In addition, the migratory capacity of EPCs significantly declined in patients with H-type hypertension(48.27 ± 14.59vs61.57 ± 12.86/mm^2, P〈0.01). Concentration of SDF-1 eytokine was declined significantly compared to controls (520.87 + 145.03vs673.77 + 168.32pg/106, P〈0.01). Conclusion The study demonstrates that numbers of EPC are decreased and functional capacities are impaired in H-type essential hypertensive patients, with is associated with the increasing concentration of homocysteine.
