摘要
目的探讨胰岛素笔一次性针头重复使用次数与皮下硬结产生的关系,寻求最适宜的针头注射次数。方法选择门诊及住院的175例使用胰岛素笔的糖尿病患者进行问卷调查,对调查结果采用Logistic回归、Pearsonχ2检验、似然比χ2检验、Fisher确切概率法进行分析。结果 175例患者中,重复使用胰岛素笔针头者134例(76.6%),仅使用1次者41例(23.4%)。重复使用组硬结发生率(56.0%)显著高于非重复组(0),差异有统计学意义(P<0.05)。胰岛素笔针头使用2次与使用3～4次比较,硬结发生率间差异无统计学意义(P>0.05);使用3～4次者硬结发生率显著低于使用5～9次者,差异有统计学意义(P<0.05);当重复使用次数达到5次以上,硬结发生率均高达60%以上,重复使用5～9次、10～19次、20～29次、≥30次两两比较,硬结发生率间无显著差异,但均显著高于使用1～4次者,差异有统计学意义(P<0.05)。结论重复使用针头会促进硬结的产生,在胰岛素注射中,针头应当只使用一次,最好不要超过4次。
Objective To investigate the relation between time （s） of use of the disposable insulin pen needles and the formation of subcutaneous nodules. Methods Totally 175 outpatient and inpatient diabetic patients who used the insulin pens were asked to complete a questionnaire. Logistic regression, Pearson X2 test, likelihood ratio chi - square test, and fisher exact probability test were used to analyze the data. Results A total of 134 out of 175 （76. 6% ） diabetic patients used the insulin pen needles for twice （4l, 23.4% ） or more. The occurrence rate of subcutaneous nodules in needle -reuse patients （56.0%） was significantly higher than that in the non -reuse patients （0） （ P 〈 0.05 ）. Twice of use did not lead to significantly different oc- currence rate compared with three or four times （ P 〉 0. 05 ） , while five to nine times of use was connected with significantly higher occurrence rate than three to four times of use. Five or more times of use was connected with over 60% occurrence rate, while five to nine times of use, 10 - 19, 20 - 29, and 30 or more times of use did not connect to significantly different occurrence rates, al- though all were higher than that of three to four times （ P 〈 0. 05 ）. Conclusion Using insulin pen needles repeatedly will lead to the formation of nodules, it is recommended to use the needles only once or at least no more than four times.
