胰岛素注射笔相对于传统注射器治疗糖尿病效果的系统评价 被引量:27

摘要 目的评估胰岛素注射笔相对于传统注射器治疗糖尿病的有效性和安全性。方法 2012年12月—2013年6月检索PubMed、EMBASE、Cochrane临床对照试验注册库、中国学术期刊全文数据库、维普中文科技期刊数据库及万方数据学术论文,检索年限为从建库至2013年1月。纳入对比分析胰岛素注射笔相对于传统注射器治疗糖尿病的临床随机对照试验,采用Review Manager 5.2软件进行Meta分析,分析胰岛素注射笔治疗组和对照组糖化血红蛋白(HbA1c)、每日胰岛素用量、患者的偏好及不良反应。结果共纳入18篇文献。胰岛素注射笔治疗组和对照组HbA1c间差异无统计学意义〔MD=-0.00,95%CI(-0.34,0.33),P=0.99〕;胰岛素注射笔治疗组和对照组每日胰岛素用量间差异无统计学意义〔MD=0.06,95%CI(-1.83,1.94),P=0.95〕;糖尿病患者对胰岛素注射笔有强烈的偏好〔OR=68.84,95%CI(23.21,204.14),P<0.000 1〕,且认为胰岛素注射笔更易操作。研究未发现胰岛素注射笔引起的严重不良事件。结论胰岛素注射笔是可行的且糖尿病患者易于接受的治疗模式。 Objective To evaluate the effectiveness and adverse effects of insulin pen versus traditional syringe in pa- tients with diabetes mellitus (DM). Methods Literatures were searched for in PubMed, EMBASE, Coehrane Central Register of Controlled Trials, Full - text Database of Chineses Academic Journal, VIP Chinese Scientific Journals and Database, Wan- fang Database. Clinical randomized controlled trials of insulin pens versus traditional syringe in treatment of DM were enrolled and analyzed comparatively. A Meta analysis was performed on glycated hemoglobin (HbAlo), daily insulin dosage, patients' pref- erence, adverse reactions in groups therapy and control. Results A total of 18 literatures were enrolled. There was no signifi- cant difference in HbAIo between two groups [ MD = - 0.00, 95% CI ( - 0. 34, 0. 33 ), P = 0.99 ], therapy group did not waste insulin as compared with control group [MD =0. 06, 95%CI ( - 1.83, 1.94), P =0. 953, DM patients intensely pre- ferred insulin pen [ OR = 68. 84, 95% CI (23. 21, 204. 14), P 〈 0. 000 1 ], thinking it easier to operate. No severe adverse reactions due to insulin pen were noted. Conclusion Insulin pen is a feasible treatment mode which is easy to be accepted by DM patients.
