摘要
目的:介绍我院强化特殊使用级抗菌药物使用管理的实践,促进临床合理用药。方法:依据《抗菌药物临床应用指导原则》等,综合制定加强抗菌药物相关知识培训、严格执行抗菌药物使用分级管理制度和《特殊使用抗菌药物申请表》审批制度等干预管理措施;借助医院信息系统,比较干预前、后各7个月内特殊使用级抗菌药物销售金额及其占抗菌药物总销售金额的比例(简称"销占比")、月销售金额。结果:特殊使用级抗菌药物销售金额从干预前的最高190万元降至100万元左右,下降幅度达47.37%;"销占比"从干预前最高的20%降至10%左右,下降幅度为50%;月销售金额从干预前的平均141.39万元下降为平均84.43万元,干预前后比较具有显著性差异(P<0.05)。结论:医院采取合理的综合干预措施,可有效降低特殊使用级抗菌药物的使用,提高临床合理应用抗菌药物的水平。
OBJECTIVE：To introduce the enhancement of clinical application management of special level antibacterial agents in our hospital,and to promote clinical use of drugs.METHODS：According to Guidance Principles for Clinical Application of Antibacterial Agents,intervention measures were established,such as strengthening antibacterial agents knowledge training,strictly implementing classification management system for antibacterial agents and approval system for Application of Antibacterial Agents for Special Use.Based on HIS,the application of special level antibacterial agents were compared in 7 months before and after intervention in respect of consumption sum,proportion of its consumption sum in total consumption sum（called ＂consumption sum ratio＂ in short） and monthly consumption sum.RESULTS：Consumption sum of special level antibacterial agents decreased from 1.9 million yuan before intervention to about 1 million yuan with decrease extent of 47.37%;＂consumption sum ratio＂ decreased from 20% to 10% with decrease extent of 50%;monthly consumption sum decreased from average 1.413 90 million yuan to average 0.844 3 million yuan,there was significant difference before and after intervention（P〈0.05）.CONCLUSION：Reasonable comprehensive intervention can effectively reduce the application of special level antibacterial agents and improve rational use of antibacterial agents in the clinic.
