摘要
目的探讨创伤性骨折微量元素含量变化与骨愈合的相关性。方法将符合入选标准的非手术椎体骨折患者随机分为治疗组和对照组,两组再根据年龄划分为儿童、成年、老年样本群。治疗组采用21金维他治疗,对照组采用安慰剂治疗,两组疗程均为12周。治疗期间两组均排除其他促进剂,抑制剂药物以及影响骨愈合的辅助治疗,不改变患者正常的膳食习惯;分别于治疗前、治疗后第1天、第3周、第8周、第12周取晨间空腹静脉血液4ml,采用原子吸收分光光度法检测患者血清中微量元素Fe、Cu、Zn、Mn含量,同期摄X线片判断骨愈合情况,并进行统计学分析。结果儿童、成年、老年三种不同年龄段治疗组及相应各对照组第3周、第8周、第12周血清微量元素Fe、Cu、Zn、Mn含量水平与治疗前比较,差异均有统计学意义(P<0.01);各治疗组分别与相应对照组组间比较,差异也有统计学意义(P<0.01)。相关性分析显示:骨痂积分与血清微量元素Fe、Cu、Zn、Mn含量有相关性。结论对微量元素代谢正常的骨折患者补充微量元素,对骨折愈合有促进作用。提示临床上在采用补充微量元素治疗骨折患者时,要考虑患者的微量元素代谢状况,有针对性地进行微量元素的干预。
Objective To investigate the correlation between changes of trace element content and bone healing in patients with traumatic fracture. Methods The non-surgical vertebral fracture patients who met the inclusion crite- ria were randomly divided into the treatment group and the control group. And then patients in these two groups were divided into children sample group, adult sample group and elderly sample group according to their ages. The treatment group was treated by Vitamains with Minerals Tablets(21) and the control group by placebo. The treatment course was 12 weeks for each group. Other accelerator,inhibitor and other auxiliary treatments that affect bone healing were eliminated. Dietary habits of patients were not changed after admission. 4 ml fasting venous blood was collected before treatment, after treatment in the morning on the first day, 3rd week, 8th week and 12th week respectively. Content of serum trace element,including Fe,Cu, Zn, Mn were tested by atomic absorption spectrophotometry. Meanwhile, the bone healing was evaluated by X-ray examination before the performance of statistical analysis. Results Intragroup comparison: there was statistical significance( P d0.01)in the content of serum trace element,including Fe, Cu,Zn, Mn in children group and adult group and elderly group on the 3rd w eek,Sth week and 12th week. The comparison be- tween the treatment group and the control group had statistical significance( P〈0.01). The analysis of correlation showed that there was correlation between callus score and content of trace element as well as Fe, Cu, Zn, Mn. Conclu- sion For fracture patients with normal trace element metabolism,supplementing trace elements had healing improve- ment effect for bone healing. It is suggested that the trace element metabolism of the fracture patients should be taken in consideration during the clinical treatment with supplementing trace elements, and trace elements should be inter- vened accordingly.
