呼吸窘迫综合征近足月患儿脑干听觉诱发电位分析

Analysis of Brainstem Auditory Evoked Potential in 39 Near-term Infants with Respiratory Distress Syndrome

摘要 【目的】探讨近足月儿伴呼吸窘迫综合征(RDS)患儿的脑干听觉诱发电位(BAEP)特点及其应用价值。【方法】对39例近足月儿伴RDS患儿进行BAEP检查,分析其潜伏期、渡间期及波幅比等数据,以30例无听力及神经损伤的近足月儿作为对照。【结果】病例组Ⅲ~V波间期、I~V波间期及各波潜伏期较对照组延长,差异有统计学意义(P〈0.01),病例组各波分化异常耳率及V/I波幅比异常耳率较对照组高,差异有统计学意义(P〈0.05);病例组各波出波耳率及I~Ⅲ间期与对照组比较差异无统计学意义(P〉0.05)。【结论】近足月儿伴RDS患儿是脑干听觉通路受损的高危人群,BAEP是早期诊断脑干听中枢功能异常和听力障碍的有效方法,对近足月儿伴RDS患儿应尽早采用BAEP动态监测,及时给予治疗和干预,从而最大限度减少神经系统后遗症的发生。 [Objective] To explore the characteristics and application value of brainstem auditory evoked potential(BAEF) in near term infants with respiratory distress syndrome(RDS). [Methods] BAEP examination was performed for 39 near-term infants with RDS. The data including the latent period, wave intervals and wave amplitude ratio were analyzed. Other 30 near-term infants without hearing or nerve impairment were chosen as controls. [Results] Compared with control group, Ⅲ-Ⅴ wave intervals, Ⅰ-Ⅴ wave intervals and latencies of various waves in case group were prolonged, and there were significant differences ( P 〈0.01). The abnormal differentiation rate of various waves and the abnormal rate of V / I wave amplitude ratio in case group were higher than those in control group, and there were significant differences( P d0.05). There was no significant difference in the appearance rate of various waves and Ⅰ-Ⅲ interval between case group and control group( P 〉0.05). [Conclusion] Near-term infants with RDS are high-risk population of brainstem au ditory pathway impairment. BAEP is an effective method of the early diagnosis of the dysfunction of brainstem auditory center and hearing disorder. Dynamic monitoring of BAEP should be performed for near-term infants with RDS as early as possible. Prompt treatment and intervention should be given so as to utmostly reduce the incidence of the sequela of nervous system.
