摘要
目的:用门控核素心室显像法测定20例正常人及47例冠心病(心绞痛36例,心梗11例)患者在静息状态下左右心室舒张功能参数,探讨左右舒张功能在冠心病患者中的意义。方法:运用放射性核素门控心室显像法分别测定正常组及冠心病组左右心室心功能,即左右室射血分数(LVEF,RVEF)、左右室最大射血率(LPER,RPER)、左右室最大充盈率(LPFR,RPFR)、左右室最大充盈时间(LTPFR,RTPFR)及左右室PFR/PER比值(LDF=LPFR/LPER,RDF=RPFR/RPER),并进行统计学分析。结果:心绞痛组LPFR、RPFR与对照组比较均P<001,静息LVEF正常占84%,有LPFR和/或RPFR降低共27例(75%);心梗组LPFR与对照组比较P<001,RPFR降低仅3/11例;病例组PFR/PER比值与对照组比较均P<005。在反映缺血性心肌损害方面,PFR较左右室EF、TPFR更为灵敏,左右室PFR的降低具有较好的一致性。结论:静息状态下,以PFR为主的左右室舒张功能降低是冠心病患者的一个重要特征,舒张功能受损早于收缩功能受损。
To explore the significance of left ventricular and right ventricular diastolic function for the patients of coronary disease.Methods:The parameters of left ventricular and right ventricular diastolic functions at quiescent status were detected with radionuclide ventriculography for 47 patients of coronary disease (36 cases with heartstroke and 11 cases with myocardial infarct) and 20 cases of normal people as the control group. In these two groups, the following parameters reflecting left ventricular and right ventricular diastolic function were detected and statistically analyzed: LVEF、RVEF、LPER、RPER、LPFR、RPFR、LTPFR、RTPFR and PFR/PER (LDF=LPFR/LPER,RDF=RPFR/RPER). Results: The patients with heartstroke had significant difference of LPFR and RPFR comparing with the control cases ( P <0.01). 84% of these patients had normal LVEF at quiescent status and 27 cases (75%) had decreased LPFR and/or RPFR. The patients with myocardial infarct had significant difference of LPFR comparing with the control cases ( P <0.01). Only 3/11 cases had decreased RPFR in this group. For all the patients, there was a significant difference of PFR/PER comparing with the control group ( P <0.05). The decrease of left ventricular and right ventricular PFR had a better consistency than other parameters, and PFR was more sensitive to reflect ischemic injury of myocardium than left ventricular and right ventricular EF、TPFR.Conclusions:For the patients of coronary disease, the decrease of left ventricular and right ventricular diastolic function mainly reflected as the decrease of PFR at quiescent status is a very important characteristic, the damage of cardiac diastolic function occurs earlier than that of cardiac systolic function.
