放射性核素心室造影法评价左心室壁瘤对右室功能的影响 被引量:1

摘要 目的利用放射性核素心室造影技术评价前壁心肌梗死后室壁瘤形成对右心室整体和局部的收缩及舒张功能的影响。方法患者分为对照组(G0),单纯前壁心肌梗死组(G1),前壁心肌梗死并发室壁瘤组(G2)。运用放射性核素心室造影检查技术测定心功能指标:①右室整体收缩功能参数:RVEF、TPE、PER、1/3EF和1/3ER。②右室整体舒张功能参数:TPF、PFR、1/3FF和1/3FR。③右室局部收缩功能参数:RVREF(6个节段)。④右室局部舒张功能参数:RVR1/3FF(6个节段)。结果右室整体收缩功能参数,RVEF,G2较G1和G0有明显下降(P<0.05)。右室整体舒张功能参数,PFR、1/3FF和1/3FR,G1和G2比G0有明显下降(P<0.05)。③右室局部收缩功能参数,RVREF在1、2节段G2较G0有明显下降(P<0.05)。在1、2节段G2较G1有明显下降(P<0.05)。④右室舒张功能参数,RVR1/3FF在1、2节段G2较G1和G0有明显下降(P<0.05)。结论左心室壁瘤的形成对右心室整体和局部的收缩和舒张功能已经构成了一定的损害。 [Objective] Using radionuclide ventriculography to study the influence of left ventricular aneurysm on right ventricular global and regional function of systolic and diastolic after anterior myocardial infarction. [Methods] Electing control group (G0), anterior myocardial infarction group (G1), anterior myocardial infarction with aneurysm (G2) to assess the parameters of global and regional right ventricular function of systolic and diastolic in the three groups using radionuclide ventriculography. The parameters included ①right ventricular global systolic function: RVEF, TPE, PER, 1/3EF, 1/3ER. ②right ventricular global diastolic function: TPF, PFR, 1/3FF, 1/3FR. ③ right ventricular regional systolic function: RVREF (6 segments). ④right ventricular regional diastolic function: RVR1/3FF (6 segment). [Results]①Right ventricular global systolic function, RVEF in G, were reduced significantly than that in G0 and G1 (P 〈0.05). ②Right ventrieular global diastolic function, the three parameters of PFR, 1/ 3FF and 1/3 FR in Gland G, were reduced significantly than that in G0 (P 〈0.05). ③Right ventricular regional systolic function, the paramenters of RVREF in G2 were reduced significantly in No. 1-2 segments than that in G0 and G1 (P 〈0.05). ④Right ventricular regional diastolic function, the parameters of RVR1/3FF in G2 were reduced significantly in No. 1-2 segments than that in G0 and G1 (P 〈0.05). [Conclusion] Forming of left ventricular aneurysm had produced damage on right ventricular global and regional function of systolic and diastolic.
