摘要
目的观察早期肠内营养对胃癌手术患者的影响,并探讨相关的护理方法及并发症的预防措施。方法选择2009年3月～2011年6月在该院接受手术治疗的84例胃癌患者,随机分为肠内营养(enteral nutri-tion,EN)组和常规处理组,每组各42例。分别于术前及术后1周测量患者体质量、上臂围度、皮肤褶皱厚度;检测患者体内前清蛋白、清蛋白、血红蛋白、总蛋白及各种免疫球蛋白的含量;记录患者肛门排气时间、住院时间及并发症的发生情况。结果 EN组患者体质量、体内前清蛋白、清蛋白、血红蛋白、总蛋白,IgG含量均高于常规处理组(P<0.05),但两组上臂围度、皮肤褶皱厚度差异无统计学意义(P>0.05);EN组肛门排气时间,住院时间均少于常规处理组(P<0.05);常规处理组有1例腹腔感染发生,肠内营养组无感染情况发生,但4例出现呕吐恶心,2例腹泻,1例腹痛,1例堵管。结论胃癌手术患者早期肠内营养可以显著提高患者的营养状况,增加机体免疫力,但应加强护理,积极防治并发症的发生。
[ Objective ] To explore the therapy efficacy of early postoperative gastric enteral nutrition and investigate the measures of nursing and complications preventing. [Methods ] Selected 84 cases of postopera- tive gastric carcinoma patients in our hospital from March, 2009 to March 2011. Then they were divided into two groups randomly, the enteral nutrition （EN） group and conventional group, 42 eases per group. The weight, thickness of rhienosis, upper-arm circumference, prealbumin, albumin, hemoglobin, total protein and immunoglobulin were detected before and on the 7 day after surgery. The flatus time, hospitalization time and complications were observed. [Results] The index of weight, prealbumin, albumin, hemoglobin, total protein and IgG of EN group was better than another （P〈0.05）. But there was no significant differences in thickness of rhicnosis and upper-arm circumference between the two groups （P 〉0.05）. The EN group take little time before flatus and discharge （P〈0.05）. There was 1 case observed abdominal infection in conventional group, but no infections were observed in EN group. While 4 cases with nausea and vomiting, 2 cases with diarrhea, lcase with abdominal pain and 1 case with plugging were observed in EN group. [Conclusion] The nutri- tional support of partial enteral nutrition during the perioperative can improve the nutritional and immunologic status, but should enhance nursing and prevent complications.
