大鼠脊髓注射罗哌卡因后脊髓组织炎性细胞因子的变化

Influence of ropivacaine to inflammatory cytokines after ropivacaine intra-spinal cord injection in rat

摘要 目的:研究对大鼠脊髓注射不同质量分数罗哌卡因后大鼠脊髓组织炎性细胞因子白细胞介素1β(IL-1β)和肿瘤坏死因子α(TNF-α)的变化,探讨罗哌卡因神经毒性的可能机制。方法:成年SD大鼠120只随机分为4组:假手术组(A组,脊髓内未注射任何药物),对照组(B组,脊髓内注射质量分数0.9%生理盐水4μL),质量分数0.5%罗哌卡因组(C组,脊髓内注射质量分数0.5%罗哌卡因4μL),质量分数2%罗哌卡因组(D组,脊髓内注射质量分数2%罗哌卡因4μL)。应用免疫组化技术检测大鼠脊髓组织注射后82、47、2 h1、和2周的细胞因子IL-1β和TNF-α免疫反应阳性细胞在脊髓损伤组织中的表达。结果:与A组比较,B、C、D组在脊髓注射后8、24、72 h、1和2周各时点的IL-1β、TNF-α蛋白表达细胞数显著增加(P<0.05),但随时间延长其阳性表达细胞数逐渐减少,至2周回落至A组水平。与B组比较,C、D组脊髓注射后8、24、72 h、1和2周各时点的IL-1β、TNF-α蛋白表达细胞数显著增加(P<0.05)。C、D两组比较无显著差异(P>0.05)。结论:大鼠脊髓注射生理盐水和罗哌卡因后脊髓组织细胞因子IL-1β、TNF-α蛋白表达细胞数显著增加,以注射罗哌卡因增加更明显。罗哌卡因的神经毒性可能与压力和成分有关,与浓度无关。 Aim:To explore effects of different concentration of ropivaeaine on expression .of inflamma-tory cytokines IL-1β, TNF-α in spinal cord injury tissue by intra-spinal injection in rat. Methods: Onehundred and twenty adult SD rats were randomly divided into four groups, sham Group( Group A, no spi-nal injection) ; Control Group( Group B, spinal injection with 0. 9% normal salt 4μL) ; Group 0. 5%Ropivacaine( Group C, spinal injection with 0. 5% ropivacaine 4μ L) ; Group 2% Ropivacaine ( GroupD, spinal injection with 2% ropivacaine 4μL). The expression of IL-1β, TNF-α in spinal cord injectedtissue were detected with immunohistochemistry at 8 h,24 h,72 h, 1 week and 2 week. Results: Com-pared with group A, the IL-1β and TNF-α protein expression were significantly increased in spinal cordinjected tissue at 8 h, 24 h, 72 h 1 week and 2 week in group B, group C and group D (P 〈 0. 05). Thepositive cells gradually decreased with time go on and closed to the Group A at 2 week. Compared withgroup B, the IL-1β, TNF-α protein expression were significantly increased in spinal cord injected tissueat 8h, 24h, 72h lw and 2w in group C and group D (P 〈0. 05). Compared with the control group, IL-1β,and TNF-α protein exspression of the experimental group A,B group and C group in spinal cord tis-sue was significantly increased, meaning there was significant difference(P 〈0. 05). There was no sta-tistically significant difference in IL-β and TNF-α protein expression between group C and group D(P 〉 O. 05 ). Conclusion :The IL-1 β and TNF-α protein expression were significantly increased in spinalcord tissue after normal salt and ropivacaine injection. Ropivacaine cause more seriously IL-1β and TNF-α protein expression than normal salt in rat spinal injection. Ropivacaine neurotoxicty may relate to injec-tion pressure and ingredients, but not relate to concentration.
