神经病理性痛大鼠鞘内注射GFAP抗体的效应及其血清GFAP的意义

摘要 目的评价神经病理性疼痛大鼠鞘内注射胶质纤维酸性蛋白(glial fibrillary acidic protein,GFAP)抗体的镇痛效应及检测其血清GFAP水平的意义。方法 18只雌性SD大鼠(250-300g)随机分为三组:正常对照组(C组,n=6)、SNI组(S组,n=6)、SNI+抗GFAP组(G组,n=6);其中S组大鼠均建立左后肢保留神经损伤性(spared nerve injury,SNI)神经病理性痛模型,G组大鼠在建立SNI模型的同时鞘内注射GFAP抗体10mg/kg。分别于SNI术前(T0)、术后1-6周(T1-T6)测定各组术侧后爪50%机械缩爪阈值(mechanical withdrawal threshold,MWT)。采用ELISA法检测各时点三组血清GFAP(sGFAP)、IL-1β(sIL-1β)和脑脊液GFAP(cGFAP)水平。结果与T0比较,S组T1-T6时50%MWT降低,sGFAP、sIL-1β和cGFAP的水平均升高,T5时达高峰,T6时开始回降(P<0.05)。与T0比较,G组T1-T6时50%MWT降低,T3-T5时sGFAP、sIL-1β和cGFAP水平升高(P<0.05),无高峰现象,并于T6时降至术前水平。C组T1-T6时各项指标的差异无统计学意义。与C组比较,S组T1-T6时50%MWT降低,sGFAP、sIL-1β和cGFAP的水平均升高,G组T1-T6时50%MWT降低,T3-T5时sG-FAP、sIL-1β和cGFAP水平升高(P<0.05)。与S组比较,G组T1-T6时50%MWT升高、T3-T6时sGFAP、sIL-1β和cGFAP水平降低(P<0.05)。结论鞘内注射GFAP抗体通过下调脊髓中的GFAP可明显缓解疼痛,通过检测血清中的GFAP水平可对痛觉异常进行预评估。 Objective To explore the effect of intrathecal glial fibrillary acidic protein(GFAP) antibody on neuropathic pain and assess the clinical significance of detection of serum GFAP in neuropathic pain rats. Methods Eighteen male SD rats weighing 250-300 g were randomly divided into 3 groups:normal control group(C group,n=6) ,spared nerve injury(SNI) group(S group,n=6) ,SNI+GFAP antibody group(G group,n=6) .The common fibular nerve and tibial nerve of the left leg were ligated and cut according to Decosterd's method for establishing the neuropathic pain model in S group.In G group,GFAP antibody was injected intrathecally when the spared nerve injury was performed.The 50% withdrawal threshold to mechanical(50% MWT) von Frey stimulation was measured before operation(T0) and at week 1-6 after operation(T1-T6) . Blood and cerebral spinal fluid(CSF) were collected for detecting the levels of GFAP and IL-1β by ELISA. Results Compared with baseline values,50% MWT reduced,and the levels of sGFAP,sIL-1β and cGFAP increased and peaked at T5,and then returned at T6 in S group(P0.05) .Compared with baseline values,50%MWT reduced at T1-T6,and the levels of sGFAP,sIL-1β and cGFAP increased at T3-T5 and returned to the baseline values at T6 in G group(P0.05) .There was no significant change in 50%MWT,sGFAP,sIL-1β and cGFAP at T1-T6 in C group(P0.05) .Compared with C group,50%MWT decreased at T1-T6 in S group and G group,while levels of sGFAP,sIL-1β and cGFAP increased at T1-T6 in S group and at T3-T5 in G group(P0.05) . Compared with S group,50%MWT decreased at T1-T6,and the levels of sGFAP,sIL-1β and cGFAP increased at T3-T6 in G group(P0.05) . Conclusion Intrathecal injection of GFAP antibody can alleviate neuropathic pain by down-regulating GFAP level in spinal cord.The serum GFAP can be used to evaluate in advance allodynia in rats.
