摘要
目的探讨联合检测血清癌胚抗原(CEA)、糖链抗原19-9(CA19-9)、C-反应蛋白(CRP)对结肠癌诊断的临床应用价值。方法分别利用电化学发光法检测CEA及CA19-9、乳胶颗粒增强免疫浊度法检测CRP在正常对照组、结肠息肉和结肠癌组中的表达量。结果与正常对照组相比较,结肠癌组CEA、CA19-9和CRP均显著升高,而结肠息肉组仅CRP有显著性升高。与结肠息肉组相比较,结肠癌组CEA和CRP均显著升高,正常对照组中仅CRP有显著性降低。用ROC曲线比较CEA、CA19-9和CRP区分结肠息肉和结肠癌的能力,发现CRP的诊断价值优于CEA和CA19-9。利用Logistic二元回归法分析联合检测三者的诊断能力,其灵敏性和特异性分别可达85.70%和95.50%,此外,将21例结肠息肉和35例结肠癌患者用于验证其灵敏性和特异性,分别为82.90%和91.40%。结论联合检测CEA、CA19-9和CRP有助于提高结肠癌的诊断价值。
Objective To explore the clinical value of combined detection of serum carcinoembryonic antigen (CEA), carbohydrate antigen 199 (CA19-9) and C-reactive protein (CRP) colon cancer. Method The ser-um CEA and CA19-9 in normal control group, patients with colon polyps and colon cancer were detected by electrochemical luminescence and the serum CRP was detected by latex particles enhance the immune turbidity assay. Results Compared with the control group, the serum CEA, CA19-9 and CRP in patients with colon cancer were significantly increased. Serum CRP in patients with colon polyps was significantly increased. Com- pared with colon polyps, the serum CEA and CRP in colon cancer group were significantly higher. While only CRP was significantly reduced in the normal control group. ROC curve was used to compare the diagnostic value of CEA, CA19-9 and CRP which were used to discriminate colon polyps and colon cancer groups. The diagnos- tic value of CRP was better than that of CEA and CA19-9. Logistic binary regression results indicated that the sensitivity and specificity of combined the detection of CEA, CA19-9 and CRP was increased to 85.70% and 95.50%. Conclusion The combined detection of serum CEA, CA19-9 and CRP may improve the diagnostic value of colon cancer.
