990株大肠埃希菌的耐药性及β-内酰胺酶基因的检测 被引量:14

摘要 目的:分析深圳市第八(宝安)人民医院产超广谱β-内酰胺酶(ESBLs)大肠埃希菌的耐药状况及基因特征,为医院感染控制及临床合理用药提供依据。方法:对我院2008—2010年间各科室送检标本所分离的大肠埃希菌进行药物敏感性试验及基因特征分析。结果:在所有990株大肠埃希菌中,检出产ESBLs大肠埃希菌576株,总检出率58.2%;其中2008年组181株、2009年组185株、2010年组210株,检出率分别为62.0%、54.1%、59.0%。,对其中56株产ESBLs大肠埃希菌PCR检测结果的分析发现TEM阳性40株、SHV阳性14株,检出率分别为71.4%和28.6%。结论:产ESBLs大肠埃希菌的检出率和耐药性都较高,实验室应加强监测,临床上应重视参照药敏结果,合理使用抗生素。产ESBLs大肠埃希菌菌株TEM和SHV基因检出率高,可能与多药耐药关系密切。 OBJECTIVE: To analyze the drug resistance and the genetic characteristics of ESBLs-producing Escherichia coli(ESBLs) for references of hospital infection control and clinical rational drug use in Shenzhen Municipal No.8(Bao'an) People's Hospital.METHODS: The Escherichia coli specimens obtained from each department during 2008-2010 period in our hospital were subjected to drug susceptibility test and genetic analysis.RESULTS: A total of 990 strains of Escherichia coli were isolated,of which,576(58.2%) were ESBLs-producing Escherichia coli,with 181 strains detected in 2008,185 in 2009 and 210 in 2010,with detection rates of 62.0%,54.1% and 59.0%,respectively over the 3 years.The PCR detection on 56 strains of ESBLs-producing Escherichia coli showed TEM positive in 40 strains(71.4%) and SHV positive in 14(28.6%).CONCLUSION: ESBLs-producing Escherichia coli showed high detection rate and high drug resistance rate,thus the monitoring of which should be tightened and the drug susceptibility test results should be taken into full consideration in rational use of antibiotics.The high detection rates of TEM and SHV genes of ESBLs-producing Escherichia coli might be related to multidrug resistance.
