摘要
目的观察利用带血管蒂胃瓣组织修复良性胆管狭窄术后胃瓣形态学和组织学改变及疗效。方法选择10例良性胆管狭窄行带血管蒂胃瓣修复手术的病例，术后胆道镜检查结合病理和影像检查。结果术后3个月胆道镜检查显示移植胃瓣黏膜形态无明显变化，病理学检查提示有轻度炎症。影像学检查显示肝外胆道形态及功能正常。随访6个月至3年，除1例因肝十二指肠韧带转移性腺癌再次手术外，其余9例术后优良率100％。结论利用带血管蒂的胃瓣修复术可作为治疗良性胆管狭窄安全可靠的手术方式之一。
Objective To investigate the morphological and histological changes of vascularized pedicle flap of stomach in repairing benign biliary stricture. Method A total 10 patients with biliary stricture were repaired with vascularized pedicle flap of stomach from January 2008 to June 2010, the morphological and histological changes of flap of stomach were by postoperative cholangioscopy and pathological examination. Results Postoperative cholangioscopy revealed the flap of stomach and ex- trahepatic bile duct can maintain normal morphology. No biliary stricture occurred in postoperative follow-up of 6 months to 3 years, except one patient received re-operation because of metastatic adenocarcinoma at hepatoduodenal ligament. Conclusion Repair of the bile duct stricture by using vascular- ized pedicle flap of stomach can preserve the biliary sphincteric function and a better long term outcome may be anticipated.
