摘要
目的研究中国北方汉族银屑病患者脂代谢紊乱情况,并探讨2型糖尿病对其的影响。方法筛选40例40岁以上中国北方汉族银屑病住院患者(银屑病组),根据有无糖尿病分组,比较两组及正常对照组的血脂变化(TG、TC、HDL-C、LDL-C)。结果与正常对照组相比,银屑病组血清TG明显升高(1.50±0.73 vs 1.03±0.32,P=0.002),TC下降(4.43±0.96 vs 4.74±0.65,P=0.048),而血清HDL-C水平明显下降(1.05±0.21 vs 1.51±0.34,P<0.001),LDL-C无明显变化(2.91±0.71 vs 2.84±0.57,P=0.671);经统计学处理,排除各种因素影响后,银屑病是否合并糖尿病组血清TG水平与正常对照组相比,差异均无统计学意义(P>0.05),银屑病合并和未合并糖尿病组血清HDL-C水平均较正常对照组明显降低(P<0.001),两组间比较,差异无统计学意义(P=1.000)。结论中国北方汉族银屑病患者体内脂代谢紊乱可能并不由2型糖尿病所致,血清TG升高可能与生活方式和其他疾病有关;HDL-C降低可能与银屑病本身疾病密切相关。
Objective To investigate the lipid profile and the influence of type 2 diabetes mellitus on the lipid metabolism in patients with psoriasis in Chinese. Methods 40 inpatients (Psoriasis group) with psoriasis who were over 40 years-old from the northern part of China, were classified into two groups based on the presence of type 2 diabetes mellitus. Serum lipids of these two groups were analyzed compared with normal control group. Results The fasting serum level of TG was higher in patients than that in controls (1.50±0.73 vs 1.03±0.32, P=0.002) , the serum value of TC was decreased (4.43±0.96 vs 4.74±0.65, P=0.048), and the serum level of HDL-C was significant lower in patients than those in controls (1.05±0.21 vs 1.51±0.34, P〈0.001) . Serum LDL-C level did not show any difference between the patients and controls (2.91±0.71 vs 2.84±0.57, P=0.671). Adopting statistical treatment to eliminate the effects of factors which could influence serum lipids, the serum TG levels of psoriatic inpatients with and without type 2 diabetes mellitus did not show any differences compared with normal control group(P〉0.05). And the serum HDL-C values of psoriatic inpatients with and without type 2 diabetes mellitus were all significantly lower than controls (P〈0.001). But there was no significant difference between the two psoriatic groups(P=1.000). Conclusion We concluded that abnormalities in lipid metabolism in patients with psoriasis could not be induced by type 2 diabetes mellitus. High TG could be associated with life style and other diseases, and low HDL-C may be closely associated with psoriasis.
