摘要
目的评价新辅助化疗对于局部晚期宫颈癌的术前疗效和手术效果。方法新辅助化疗组32例局部晚期宫颈癌术前进行2个周期的TP方案化疗,化疗结束后2~3周行广泛全子宫切除加盆腔淋巴结清扫术,并以同期相同条件的直接手术的32例局部晚期宫颈癌作为对照,观察化疗前后肿瘤大小、SCC值的变化和化疗毒副反应,并对两组手术情况和术后病理进行比较分析。结果新辅助化疗组患者化疗后肿瘤明显缩小（P〈0.05）,其中3例达到CR,20例达到PR,总有效率为71.88%;SCC值也明显下降（P〈0.05）;患者对化疗毒副反应均能耐受。与对照组相比,手术时间明显缩短,出血量明显减少,淋巴结阳性率、脉管受侵率及宫旁累及率显著降低（P〈0.05）。结论局部晚期宫颈癌术前新辅助化疗安全有效,有利于肿瘤切除,可提高患者生活质量。
Objective To investigate the preoperative and operative effects of neoadjuvant chemotherapy in the treatment of patients with locally advanced cervical cancer.Methods Two cycles of TP regimen chemotherapy were given to the 32 patients with locally advanced cervical cancer before the operation in the neoadjuvant chemotherapy group,then radical hysterectomy and pelvic lymphadenectomy were given for 2-3 weeks later.The other 32 patients with the same condition received primary surgery as the control group.The tumor size,the change of SCC and the chemotherapy toxicities were observed before and after chemotherapy,and the situation of operation and the postoperative pathology were compared between the two groups.Results The tumors were smaller significantly after chemotherapy in the neoadjuvant chemotherapy group（P〈0.05）,among the 32 patients,the 3 cases got CR,the 20 cases got PR,the total response rate was 71.88%;SCC decreased significantly（P〈0.05）;all the patients could tolerate the chemotherapy toxicities.Compared with the control group,the operation time was shortened significantly,the amount of bleeding was reduced significantly,the rate of lymph node metastasis,the invasion rate of intravascular and the involvement rate of parametrium decreased significantly in the neoadjuvant chemotherapy group（P〈0.05）.Conclusion Preoperative neoadjuvant chemotherapy in the treatment of locally advanced cervical cancer is safe and effective,it can do the benefit to the tumor resection,and improve the quality of life of the patients.
