摘要
目的:评估肥胖指标腰围(WC)腰臀比(WHR)、体重指数(BMI)与血脂素乱的相关性,计算三个指标预测血脂紊乱的筛选切点。方法:共纳入河北省衡水市高等院校男女教职工4 000名,年龄≥16岁。WC、WHR、BMI预测血脂紊乱的ROC曲线下面积(95%CI)通过软件统计。各肥胖参数与血脂紊乱相关性的多因素分析采用Logistic回归。结果:3525名参与数据完整纳入研究,各个肥胖指标鉴别血脂紊乱的最大敏感性和特异性的ROC下面积均有统计学意义。三个指标与总的血脂紊乱的相关性优劣没有统计学关、差异(P>005)。亚组分析显示:男性WC与TC和LDL相关性的ROC下面积更大,女性WHR与TG的相关性ROC下面积更大(P<0.05)。结论:无论男性还是女性,WC、WHR、BMI与血脂紊乱均具有明显的相关性。男性WC与TC和LDL相关性更好,女性WHR与TG的相关性更好。
Objective:To assess which of the three measures(Body mass index (BMI), waist circumference (WC) and waist-tohip ratio(WHR)) is more informative at the population level to identify individuals with dyslipidemia and to valuate the cutoff point of the best predicted value of dyslipidemia.Methods:The study population who came from Hengshui senior University included 4 000 faculty aged≥16 ys recruited in an Health Center of Harriceson International Peace Hospital attached to Hebei Medical University in 2009.The areas under the ROC curves of BMI. WC. WHR and their 95% CI were computed and compared.Logistic regression analysis showed that multiple factors modified the discriminant ability ofBMI. WC. WHR in identifying dyslipidemia.Results:3 525 subjects aged 21-91 ys including 2 012 males and 1 513 females were enrolled in the study.Highest Sensitivity and specificity of 3 parameters in identifying dyslipidemia showed significantly statistical difference.There was no statistical difference among 3 paremeters in indentifying the onset risk of dyslipidemia.Furthermore there were greater the area of ROC of WC in indentifying the onset risk of TC and LDL in males.also . WHR in indentifying the onset risk of TG in females(P〈0.05).Conclusion:BMl. WC and WHR are all strongly linked to the individuals with dyslipidemia and are all suited to screen civil servants with dyslipidemia in both male and female.Otherwise.Waist circumference,a practical tool that had a higher discriminant ability than BMI and WHR in identifying males with or without TC and LDL.also WHR,had a higher discriminant ability than BMI and WC in identifying females with or without TG,appears as the best screening tool in SEX specially.
