摘要
目的:为无痛肠镜选择一个安全、操作性强的麻醉方法。方法:选择300例符合纳入标准的需作无痛肠镜的患者随机分成三组,单纯丙泊酚组(P组)、布托啡诺复合丙泊酚组(B组)、舒芬太尼复合丙泊酚组(S组),每组100例;P组先静脉注射0.9%生理盐水0.1ml/kg,3分钟后缓慢静注丙泊酚至意识消失,并使用丙泊酚维持麻醉;B组先静注布托啡诺0.1ml/kg(布托啡诺浓度为0.1mg/ml),3分钟后缓慢静注丙泊酚至意识消失,并使用丙泊酚维持麻醉;S组先缓慢静注舒芬太尼0.1ml/kg(舒芬太尼浓度为0.1μg/ml)3,分钟后缓慢静注丙泊酚至意识消失,并使用丙泊酚维持麻醉。结果:B组、S组镇痛效果、循环抑制、术后恢复时间明显优于P组,术中呼吸抑制三组间P组最明显、S组次之、B组最轻。结论:布托啡诺复合丙泊酚组在无痛肠镜检查的麻醉中更安全,更适用。
Objective:To select a safe and workable method of anesthesia for colonoscopy.Methods: 300 patients with the admitted criteria and requiring for the painless colonoscopy were randomly divided into three groups:propofol group(P group),butorphanol plus propofol group(B group) and sufentanil plus propofol group(S group).Every group has 100 patients.In P group,patients were firstly given intravenous injection of 0.9% saline,3 minutes later,intravenous injection of propofol 0.1ml/kg until losing the consciousness and then anesthesia was maintained with propofol;In Group B,patients were firstly given butorphanol 0.1ml/kg(consistence of butorphanol is 0.1mg/ml),3 minutes later,intravenous injection of propofol until losing the consciousness and then anesthesia was maintained with propofol;In S group,patients were firstly given butorphanol 0.1ml/kg(consistence of sufentanil is 0.1ug/ml),3 minutes later intravenous injection of propofol until losing the consciousness and then anesthesia was maintained with propofol.Results: Antinociceptive effects of B group and S group were significantly better than those of P group.Circulatory inhibition of B group and S group was significantly less serious than that of P group;Time of postoperative resuscitation of B group and S group was markedly shorter than that of P group;Respiratory inhibition during operation was the most common in groups P,followed by group S,which is the least serious in B group.Conclusion:Butorphanol plus propofol anesthesia in painless colonoscopy is safer and more applicable.
