摘要
目的通过观察三氧化二砷术前短期体内化疗对大肠癌细胞分子生物学的影响,探讨三氧化二砷抗肿瘤作用的机制,观察短期术前化疗后早期分子生物学指标的变化,评价三氧化二砷在人体内的化疗效果,为其在临床应用于大肠癌的治疗提供依据。方法取消化内镜检查及活检诊断为大肠腺癌且计划进行手术切除的患者20例,随机分为两组,分别为实验组和空白对照组(每组10例),保存患者术前行内镜诊断时所取得的癌组织。实验组:亚砷酸注射液10 mL加入5%葡萄糖注射液500 mL中,术前1次/d,静脉滴注,连用3 d;空白对照组:不用药。两组均于第4 d复查血常规、肝、肾功能后手术。取术后病理癌组织。将患者术前与术后所取标本进行如下处理:固定、包埋、HE染色,检测肿瘤细胞形态和免疫组化法检测P53、bcl-2蛋白表达情况。结果统计学分析表明,As2O33 d化疗前后大肠癌细胞凋亡指数,差异显著(P<0.05)。化疗前后大肠癌细胞P53蛋白表达阳性率差异不具有统计学意义(P>0.05)。化疗前后大肠癌细胞bcl-2蛋白表达率差异具有统计学意义(P<0.05)。结论 As2O3短期体内化疗可以诱导大肠癌细胞凋亡。可能是通过下调bcl-2蛋白表达而实现的。
Objective To observe the influence of chemotherapy in vivo with trioxide arsenic to colorectal cancer on molecular biology,study the chemotherapeutic mechanism of trioxide arsenic,evaluate the chemotherapeutic effects,and search for new clinical methods of trioxide arsenic to treat patients with colorectal cancer.Methods 20 patients,who have been finally diagnosed as colorectal adenocarcinoma through endoscopy and biopsy,were divided into two groups randomly,the test group and the blank control group.10 patients per group,and the malignant tissues of them were studied by endoscopy and biopsy.Arsenious oxide injection 10 mL with 5% Glucose 500 mL,qdivgtt,3 d,were given in the test group,and no treatment in the blank control group.All the patients receive operations the day after that.Take the malignant tissue during the operation.The tissues were dealed with such as: fixing、embedding、HE staining、morphocytologic observation and immunohistochemical examination of expression of P53、bcl-2 protein.Results Statistics analysis showed that the difference of apoptosis index of colorectal cancer between pre-chemotherapy and post-chemotherapy was significant(P0.05).Difference of positive percentage of P53 protein expression between pre-chemotherapy and post-chemotherapy was not significant(P0.05).Difference of positive percentage of bcl-2 protein expression between pre-chemotherapy and post-chemotherapy was significant statistically(P0.05).Conclusion Short-term chemotherapy in vivo with trioxide arsenic to colorectal cancer might be induce apoptosis of malignant cells by way of down regulating the expression of bcl-2 protein possibly.
