摘要
目的探讨艾滋病病毒Ⅰ型(HIV-1)抗体蛋白印迹法(WB)在性病门诊就诊者中的应用情况。方法用第3代HIV酶联免疫吸附试验(ELISA)试剂筛查11 558份性病门诊就诊者血浆样本,ELISA阳性样本和ELISA阴性但反转录聚合酶链反应(RT-PCR)法阳性样本,用WB确证;RT-PCR法检测筛查结果阴性样本、WB结果可疑和阴性样本、以及部分WB阳性样本,进行RT-PCR检测。结果 18份WB阳性样本中,17份RNA阳性,1份RNA阴性;14份WB可疑样本中,13份RNA阴性,1份RNA阳性;32份WB阴性样本中,4份RNA阳性,28份RNA阴性。结论 (1)HIV WB存在生物学假阳性。(2)检测可疑者样本中HIV RNA,减少随访负担。(3)WB存在窗口期,不能检出HIV感染急性期样本。
Objective To investigate the application of Western blot(WB) for HIV testing in STD clinics.Method A total of 11 558 specimens were screened by 3rd generation HIV ELISA assay,the HIV-positive samples by ELISA were confirmed by WB test,and the others were tested by RT-PCR;samples which were HIV-negative by ELISA but positive by RT-PCR were confirmed by WB test,and the HIV-negative or indeterminate samples and part of positive samples by WB were re-test by RT-PCR.Results Of the 18 blood samples,17 positive by RT-PCR were positive by WB;and among 14 indeterminate blood samples by WB only one was positive by RT-PCR;among 32 specimens which were negative by WB 4 was positive by RT-PCR.Conclusion 1) There was biological false positive reaction in WB confirmatory test.2) Application of HIV RNA assay to test indeterminate samples can reduce the burden of follow up.3) WB test could not effectively confirm the acute HIV infected sample owing to its window period.
