经皮球囊扩张椎体后凸成形术治疗老年骨质疏松性椎体压缩骨折 被引量:12

摘要 目的探讨经皮球囊扩张椎体后凸成形术(percutaneous kyphoplasty,PKP)治疗老年骨质疏松性椎体压缩骨折的临床效果。方法 2005年1月至2008年12月应用经皮球囊扩张椎体后凸成形术治疗老年骨质疏松性椎体压缩骨折患者36例(48个椎体),所有手术均在"C"型臂X射线机引导下进行。术前及术后分别进行视觉模拟疼痛评分(visual analog scale,VAS)、椎体高度及Cobb’s角测量。结果 36例患者术后疼痛均得到有效控制。椎体高度较术前明显恢复,术前压缩椎体的前、中缘相当于椎体后缘高度的(56.10±14.59)%和(59.86%±19.64)%,术后恢复到(82.36±26.54)%和(86.72±23.26)%。椎体Cobb’s角由(10.1±3.2)°到(6.3±3.4)°。术前VAS评分为(8.7±1.2)分,术后一周降低到(1.4±0.8)分,差异有统计学意义(P<0.01)。结论 经皮球囊扩张椎体后凸成形术手术创伤小,手术时间短,可迅速有效地缓解疼痛,是治疗老年骨质疏松性椎体压缩性骨折的安全有效方法。 Objective To explore the clinical effect of percutaneous kyphoplasty(PKP) in the treatment of aged osteoporotic vertebral compression fracture.Methods From January 2005 to December 2008,36 patients(48 vertebrae) with aged osteoporotic vertebral compression fracture were treated by PKP.All the operations were performed under the guidance of C arm X-ray machine.The clinical effect was evaluated by observing the charges of visual analog scale(VAS),height of vertebral bodies and Cobb's angle.Results All cases had rapid,significant and sustained improvement in back pain following PKP.Vertebral body height significantly recovered after operation.The anterior and midline vertebral body height was equivalent to(56.10±14.59)% and(59.86±19.64)% of posterior vertebral body respectively,and the proportion increased to(82.36±26.54)% and(86.72±23.26)% after operation.The Cobb's angle improved from(10.1±3.2)° preoperatively to(6.3±3.4)° postoperatively.(VAS)decreased from 8.7±1.2 preoperatively to 1.4±0.8 postoperatively.Conclusions The PKP technique has advantages of little hurt and short operation time.It can relieve the pain rapidly and effectively.It's safe and effective for aged osteoporotic vertebral compression fracture.
