摘要
目的探讨多层螺旋CT尿路成像(MSCTU)在诊断膀胱疾病中的应用价值.方法对30例怀疑泌尿系统梗阻性疾病患者进行常规泌尿系统平扫加多期增强CT扫描,待膀胱充盈造影剂满意后,再行膀胱区扫描,对增强后图像进行后处理获得CTU图像.后处理技术包括多平面重组(MPR)、曲面重组(CPR)、容积再现(VR)及最大密度投影(MIP)等.结果 30例患者中膀胱肿瘤10例;先天畸形5例;膀胱结石6例;膀胱结核6例;前列腺增生2例;膀胱憩室1例.CTU图像通过CT轴位及后处理技术,可以形象、直观、多角度观察膀胱肿瘤腔内形状,对肿瘤在膀胱壁的浸润范围也可清晰显示,对膀胱其他病变的检出和定性也具有较好的应用价值.结论 CTU后处理技术可清晰直观地显示泌尿系统的解剖结构,结合常规轴位图像可准确显示膀胱病变的部位、形态、大小、性质及与周围组织的解剖关系,可作为泌尿系梗阻性疾病的首选检查方法之一.
Objective To investigate the application value of MDCT urography(MSCTU) in diagnosis of bladder disease.Methods 30 patients suspected to have urography obstruction diseases underwent urography system regular CT plain scanning and multiple phases-enhanced scanning.After the bladder was filled with the contrast,CT scanning was performed on bladder area,and then post-processings,including multiple planar reconstruction(MPR),curved planar reconstruction(CPR),volume representation(VR) and the maximum intensity projection(MIP) which were followed on the enhanced imaging to get CTU image.Results In 30 patients,10 cases were diagnosed as bladder carcinoma,5 cases were congenital malformation,6 cases were bladder stone,6 cases were bladder tuberculosis,2 cases were prostate hyperplasia,and 1 case was bladder diverticula.CTU image through CT axial scan and post-processing technology could observe the shape of bladder tumor in bladder vividly,intuitively and multi-perspectively,could display the invasion of the bladder wall scope clearly,and also had good application in the detection and qualification of other bladder disease.Conclusion CTU post-processing technology can clearly and intuitively show the anatomy of urinary system,combining general CT axial scan images whicl can accurately show the position,shape,size,nature and anatomic relationship with the surrounding tissue of bladder lesions.So it can be used as the primary examination method for urinary obstruction disease.
