摘要
目的观察膏苓解毒泄浊方联合西医综合治疗痰热壅肺型慢性阻塞性肺疾病(COPD)急性加重期(AECOPD)的临床疗效。方法将56例痰热壅肺型AECOPD患者随机分为2组,对照组26例予西医综合治疗,治疗组30例在对照组治疗基础上加膏苓解毒泄浊方治疗。1周后,对2组治疗前后呼吸困难评分进行比较;进行血气分析,测定2组血氧分压[p(O2)]、血二氧化碳分压[p(CO2)];肺通气功能,主要分析第1 s用力呼气容积占预计值百分比(FEV1%);观察临床疗效。结果 2组治疗后呼吸困难评分、FEV1%及p(O2)均较本组治疗前有明显改善(P<0.05),且2组间比较差异有统计学意义(P<0.05)。2组治疗后p(CO2)与本组治疗前比较明显改善(P<0.05)。治疗组总有效率96.7%,对照组总有效率69.2%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。结论膏苓解毒泄浊方联合西医综合治疗痰热壅肺型AECOPD可有效恢复患者肺通气功能,改善患者呼吸困难及缺氧症状,减轻患者痛苦,降低死亡率,疗效确切。
Objective To observe the clinical effect of acute exacerbation chronic obstructive pulmonary disease with phlegm-heat obstructing in the lung type treated by combination of Gaolingjieduxiezhuo recipe and western medicine.Methods 56 cases of acute exacerbation chronic obstructive pulmonary disease with phlegm-heat obstructing in the lung type were randomly divided into two groups.Patients in control group were treated by comprehensive treatment of western medicine.Patients in treatment group were treated by combination of Gaolingjieduxiezhuo recipe and western medicine.The dyspnea scores before and after treatment was compared in two groups. and [p(CO2)] were measured by blood gas analysis.The pulmonary ventilation function was evaluated by FEV1%.Results The dyspnea score,FEV1% and after treatment was improved in comparison with those before treatment in two groups(P0.05).There was significant difference between two groups(P0.05).[p(CO2)] after treatment was improved in comparison with that before treatment in two groups(P0.05).The total effective rate in treatment group(96.7%) was higher than that in control group(69.2%,P0.05).Conclusion Gaolingjieduxiezhuo recipe combined with comprehensive treatment of western medicine can effectively restore p pulmonary ventilation function in patients,improve shortness of breath,hypoxia symptoms,and reduce patient suffering,mortality.
