B-flow联合时间-空间相关成像技术在正常胎儿心血管系统中的应用 被引量:5

摘要 目的评价四维B-flow联合时间-空间相关成像(STIC)技术在正常胎儿心血管系统中的临床应用价值。方法选择306胎孕20~39周正常胎儿,先在二维超声下进行常规扫查,然后进行B-flow显像及STIC数据采集。结果在306胎中,289胎在B-flow显像方式下STIC数据采集成功,采集成功率为94.44%(289/306);所采集的容积数据通过重建可获得下列心外血管:主动脉弓、动脉导管弓、降主动脉、下腔静脉、静脉导管、肝静脉及肺静脉。B-flow联合STIC技术对≥1条、≥2条、≥3条和4条肺静脉的显示率分别为100%(289/289)、97.23%(281/289)、81.31%(235/289)、46.37%(134/289),彩色多普勒超声的显示率分别为100%(289/289)、94.46%(273/289)、44.64%(129/289)、17.65%(51/289),二维灰阶超声显示率分别为100%(289/289)、93.08%(269/289)、40.83%(118/289)、15.92%(46/289)。采用B-flow联合STIC技术对于3条及以上肺静脉和4条肺静脉的显示率明显高于二维灰阶超声与彩色多普勒超声(P均〈0.05)。结论 B-flow联合STIC技术能较好地显示血流速度较低的细小血管,如肺静脉。 Objective To observe the application value of four-dimensional(4D) B-flow spatio-temporal image correlation(B-flow-STIC) in visualizing fetal cardiovascular system.Methods Three hundreds and six normal single fetuses from 20 to 39 weeks of gestation were examined with 2D transducer.Then fetal cardiac blood flow was acquired with B-flow-STIC images.The cardiac vascular system was analyzed off-line on multiplanar and reconstructed 4D rendered images.Results Complete B-flow-STIC collection was obtained in 289 fetuses(289/306,94.44%).All extracardiac vessels of aortic arch,ductus arteriosus arch,descending aorta,inferior vena cava,ductus venosus,hepatic vein and pulmonary veins could be detected on reconstructed 3D and 4D images.The displaying rate of pulmonary veins ≥1,≥2,≥3 and 4 with B-flow-STIC on was 100%(289/289),97.23%(281/289),81.31%(235/289) and 46.37%(134/289),with color Doppler ultrasound was 100%(289/289),94.46%(273/289),44.64%(129/289) and 17.65%(51/289),with 2D ultrasound was 100%(289/289),93.08%(269/289),40.83%(118/289) and 15.92%(46/289),respectively.B-flow-STIC was better than color Doppler ultrasound and 2D ultrasound in display of pulmonary veins ≥3 and 4(all P0.05).Conclusion B-flow-STIC allows visualization of cardiac blood vessel including fine small vessels with low velocity,such as pulmonary veins.
