麝香保心丸对心力衰竭大鼠心脏血管紧张素受体的影响 被引量:10

The effects of Shexiangbaoxin pills on the expression of cardiac angiotesin receptors in rat hearts with heart failure

摘要 目的:探讨麝香保心丸对心肌梗死后心力衰竭(心衰)大鼠血浆肾素活性(plasma renin ac-tivity,PRA)、血浆血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)水平的影响、对非梗死区左心室组织血管紧张素Ⅱ1型受体(angiotensinⅡtype 1 receptor,AT1R)和血管紧张素Ⅱ2型受体(angiotensinⅡtype 2 receptor,AT2R)信使RNA(messenger ribonucleic acid,mRNA)表达水平的影响。方法:结扎大鼠左冠状动脉前降支制作心肌梗死后心衰模型,54只Wistar大鼠随机分为正常对照组(A组)、假手术组(B组)、心衰模型对照组(C组)、阳性药物对照组(D组)、麝香保心丸小剂量组(E组)和麝香保心丸大剂量组(F组),采用灌胃法给药。超声心动图检测用药前后大鼠心脏功能,放射免疫法测定血浆PRA、AngⅡ水平,半定量逆转录聚合酶链反应(reverse transcription polymerase chain reaction,RT-PCR)法检测大鼠非梗死区左心室组织AT1R、AT2R的mRNA表达水平。结果:1.用药前,A组与B组比较,大鼠左心室射血分数(leftventricular ejection fraction,LVEF)组间差异无统计学意义(P>0.05);与B组比较,C、D、E及F组大鼠LVEF明显降低(P<0.01)。用药后,与C组比较,F组LVEF显著改善(P<0.05),D组、E组LVEF有升高趋势,但差异无统计学意义。2.A组和B组大鼠的血浆PRA、AngⅡ水平,以及心肌AT1R、AT2RmRNA表达水平组间比较差异无统计学意义(P>0.05);与B组比较,C组大鼠PRA、AngⅡ及AT2R水平均明显升高(P<0.01),而AT1R表达明显降低(P<0.01);与C组比较,D、E、F组PRA、AT2R均显著降低(P<0.05和P<0.01);D、F组AngⅡ显著降低(P<0.05),AT1R水平显著升高(P<0.01)。结论:应用麝香保心丸治疗升高了LVEF,改善了心肌梗死后心衰大鼠的心脏功能,由于心肌血供改善,降低了循环中PRA、AngⅡ水平,同时使慢性心力衰竭(CHF)时心肌局部的AT1R、AT2R表达趋于正常,从而对CHF起到了有益的治疗作用。同时,与小剂量麝香保心丸相比,大剂量疗效更为显著。 Objective:To investigate the effect of Shexiangbaoxin Pills(SXBXP) on plasma renin activ-ity(PRA) and angiotensin Ⅱ(AngⅡ) plasma levels of chronic heart failure(CHF) rats,and on expression of Angiotesin ⅡType 1 Receptor(AT1R) and Angiotesin ⅡType 2 Receptor(AT2R) in the messenger RNA(mR-NA) levels of non-infarction left ventricular tissue from rats with CHF induced by myocardial infarction.Meth-ods:Models of CHF established by left anterior descending coronary artery ligature.Fifty four Wistar rats were randomly divided into five groups: normal control group(group A),sham operation group(group B),CHF model group(group C),positive medicine control group(group D),small dose SXBXP group(group E) andlarge dose SXBXP group(group F),deployed intragastrically.Cardiac function was examined by echocardio-graphy before and after therapy,PRA and Ang Ⅱ plasma levels were tested by radio-immunity,mRNA ex-pressed levels were measured by semiquantitative reverse transcription polymerase chain reaction(RT-PCR) for AT1R and AT2R in non-infarction left ventricular tissue.Results:(1) There was no significant difference in left ventricular ejection fraction(LVEF) between groups A and B.Compared with group B,LVEF of groups C,D,E,F were significantly decreased(P〈0.01) before therapy.After therapy,compared with group C,LVEF of group F was significantly improved(P〈0.05).LVEF of groups D and E increased even though there was no significant difference were detected compared with group C.(2)Plama levels of PRA,AngⅡ,and mRNA expression levels of myocardial AT1R,AT2R in groups A and B appeared no diference(P〉0.05).Compared with group B,PRA,AngⅡand AT2R expressed levels were markedly increased(P〈0.01),AT1R were signif-icantly decreased(P〈0.01) in group C.Compared with group C,levels of PRA and AT2R were significantly declined in groups D,E and F(P〈0.05 and P〈0.01).AngⅡ was markedly declined(P〈0.05) and AT1R was significantly increased(P〈0.01) in groups D and F.Conclusion: After SXBXP treatment,LVEF is in-creased and cardiac function is significantly ameliorated in rats with CHF.The therapeutical effect of SXBXP may be related to better blood supply for myocardium and down-regulation of PRA and AngⅡplasma levels,and regulation of AT1R and AT2R inclined to normal.The results show that that SXBXP can be used in treatment of CHF and the therapeutical effect of large dose SXBXP is superior to small dose SXBXP.
