儿童肺炎链球菌对青霉素耐药性分析 被引量:16

摘要 目的了解乐清地区儿童感染肺炎链球菌对青霉素及其他10种抗菌药物的耐药性。方法收集2007年1月-2009年12月,医院儿科门诊及入院首日患儿送检的呼吸道标本中分离出的201株肺炎链球菌药敏结果进行回顾性分析。结果 2007-2009年肺炎链球菌对青霉素的耐药率逐年升高,分别为71.0%、77.6%和83.8%,除青霉素外,对磺胺甲噁唑/甲氧苄啶、克林霉素、红霉素、四环素的耐药率高,分别为88.6%、93.0%、98.5%、94.5%,对万古霉素和左氧氟沙星均敏感;肺炎链球菌对青霉素敏感组与青霉素非敏感组的药物敏感性比较显示,两组对阿莫西林、头孢噻肟、磺胺甲噁唑/甲氧苄啶、克林霉素、红霉素和四环素的耐药性差异有统计学意义,特别是克林霉素、红霉素和四环霉素在两组的耐药率均>65.0%,且在青霉素非敏感组这种耐药性更加显著。结论乐清地区的儿童感染肺炎链球菌,青霉素的耐药逐年升高,磺胺甲噁唑/甲氧苄啶、克林霉素、红霉素、四环素的耐药率高,耐药情况严峻。 OBJECTIVE To learn about the drug resistance of children infected with Streptococcus pneumoniaeto penicillin and other 9 antibiotics in Yueqing region.METHODS A retrospective analysis was conducted on drug sensitive results of 201 strains of S.pneumoniaeisolated from respiratory tract of children submitted from pediatric outpatient clinic of the hospital and the first day of hospitalization from Jan 2007 to Dec 2009.RESULTS The drug resistance rate of S.pneumoniaeto penicillin in three years from 2007-2009 were 71%,77.6% and 83.8%,respectively.In addition to penicillin,the drug resistance rate of compound sulfamethoxazole,clindamycin,erythromycin and tetracycline were very high,they were 88.6%,93.0%,98.5% and 94.5%,respectively,and S.pneumoniaewas sensitive to both of Vancomycin and Levopropoxyphene.The comparison between drug sensitivity of S.pneumoniaeto penicillin-sensitive group and non-sensitive group indicated that the two groups were statistically significant on drug resistance comparison difference to amoxicillin,cefotaxime,cotrimoxazole,clindamycin,erythromycin and tetracycline.Particularly,all resistance rates of clindamycin,erythromycin and tetracycline in the two groups were greater than 65.0%,the drug resistance in penicillin non-sensitive group was more significant.CONCLUSION The drug resistance of penicillin is increasing year by year among children infected with S.pneumoniaein Yueqing region,the drug resistance rates of compound sulfamethoxazole,clindamycin,erythromycin and tetracycline are high,and the drug resistance situation is severe.
