口腔癌及癌前病变nm23-H1肿瘤抑制基因与CD44v的表达研究

摘要 目的 探讨肿瘤抑制基因nm23- H1 与CD44 变异型(CD44v) 在口腔癌中的作用和意义。方法采用流式细胞术,对肿瘤转移抑制基因nm23- H1 和CD44v 的表达量进行定量分析。结果nm23 - H1 抑制基因在口腔癌、白斑、扁平苔癣中表达的阳性率分别为55 % ,10 % 和7 % ;CD44v 在口腔癌、白斑、扁平苔癣中表达的阳性率分别为75 % 、30 % 和21 % ;在nm23- H1 基因的平均表达量中,口腔癌与白斑以及与扁平苔癣之间差异均有显著性( P< 0 .01 和P< 0.005);在口腔癌中,nm23 - H1 阴性表达的转移率为28 % ,高于nm23 - H1 阳性表达的转移率5 % ,二者之间差异有显著性( P< 0 .02);在CD44v 的平均表达量中,口腔癌与口腔正常粘膜以及与白斑和扁平苔癣之间差异均有显著性( P < 0 .005 和P < 0.001) ;在口腔癌中,CD44v 阳性表达的转移率为20 % ,高于CD44v 阴性表达转移率0,两组间差异有显著性( P < 0.01)。结论在口腔癌前病变发展为口腔癌的过程中,nm23 - H1 抑制基因和CD44v 均有显著表达;nm23 - H1 的低表达和CD44v 的高表? ObjectiveTo explore the significance and role of tumor metastatic suppressor gene nm23-H1 and CD44 variant (VD44v)in tumorigenesis of the oral squamous cell carcinoma (SCC). Methodsnm23-H1 gene protein and CD44v protein expression in oral SCC and oral premalignant lesions were quantitively studied by flow cytometry. Resultsnm23-H1 expression was found in 55 % ,10 % and 7 % of oral SCC,leukoplakia and lichen planus, respectivly.CD44v expression was detected in 75 %,30 % and 21 % of the above diseases, respectively. The statistical significances of the fluorescence indexes(FI) in nm23-H1 expression were found between oral SCC and leukoplakia as well as lichen planus (P< 0.01 and P< 0.005 , respectively) in oral SCC.nm23-H1 lower expression tumors were associated with a higher rate of metastasis(28 %). The obvious different statistical significances of FI in CD44v expression were found between oral SCC and normal mucosa, leukoplakia as well as lichen planus (P< 0.05 , P< 0.005 and P< 0.001 ,respectively).CD44v higher expression tumors were associated with a higher rate of metastasis (20 %). ConclusionThere was obvious alteration of nm23-H1 and CD44v in the process of leukoplakia and lichen planus to oral SCC.nm23-H1 lower expression and CD44v higher expression may have an important significance in the metastasis of oral SCC.
