摘要
目的:探讨Survivin蛋白在宫颈癌组织中表达及其与肿瘤增殖、微血管密度计数、肿瘤细胞凋亡的关系和临床意义。方法:采用免疫组化SP法和DNA原位未端标记TUNEL法分别测定45例宫颈癌(CC)组织和10例正常宫颈(NC)组织中Survivin、Ki-67、CD34的表达和细胞凋亡。结果:Survivin蛋白在CC中的阳性表达为68.89%,与病理分级、临床分期和淋巴结转移密切相关(P<0.05),正常宫颈组织中无阳性表达。Ki-67标记指数(LI)在CC及NC中分别为(14.12±5.19)%和(7.32±2.98)%,其差异有统计学意义(P<0.05)。CC中MVD计数33.24±9.17显著高于正常宫颈组织17.63±5.54,两者之间差异有统计学意义(P<0.05)。CC组织及NC组织中细胞凋亡指数(AI)分别为7.55±2.35、5.64±1.81,其差异有统计学意义(P<0.01)。Survivin蛋白的表达与Ki-67 LI、MVD计数呈正相关(r分别为0.477和0.386,P<0.05);Survivin蛋白的表达与细胞凋亡指数呈负相关(r=-0.541,P<0.01)。结论:Survivin蛋白的异常表达而引起细胞凋亡抑制或逃避、肿瘤细胞增殖和微血管增生增加,在宫颈癌的发生、发展中起一定的作用;Survivin蛋白检测有助于对肿瘤细胞的分化程度、病理分期作出正确评价,以指导临床治疗及估计预后。
Objective:To explore the relationship between expression of Survivin protein in cervical cancer tissues and tumor proliferation,microvessel density(MVD) and apoptosis of tumor and its clinical significance.Methods:Immunohistochemical SP method and TUNEL method were used to detect the expressions of Survivin,Ki-67 and CD34 and apoptosis in 45 samples of cervical cancer tissues and 10 samples of normal cervical tissues.Results:The positive rate of Survivin protein in cervical cancer tissues was 68.89%,which was related to pathological grades,clinical stages and lymph node metastasis closely(P〈0.05);Survivin protein did not express in normal cervical tissues;the labeling indexes of Ki-67 in cervical cancer tissues and normal cervical tissues were(14.12±5.19)% and(7.32±2.98)%,respectively,there was significant difference(P〈0.05);MVD in cervical cancer tissues was(33.24±9.17),which was significantly higher than that in normal cervical tissues(17.63±5.54)(P〈0.05);the apoptosis indexes in cervical cancer tissues and normal cervical tissues were(7.55±2.35) and(5.64±1.81),respectively,there was significant difference(P〈0.01);there was positive correlation between expression of Survivin protein and labeling index of Ki-67,MVD(γ=0.477 and 0.386,respectively,P〈0.05);there was a negative correlation between expression of Survivin protein and apoptosis index(γ=-0.541,P〈0.01).Conclusion:The abnormal expression of Survivin protein plays an important role in occurrence and development of cervical cancer by inducing apoptosis inhibition or escape,proliferation of tumor cells and increase of microvessel proliferation;detection of Survivin protein is helpful to evaluate the degree of cell differentiation and pathological stages correctly,in order to direct clinical therapy and estimate prognosis.
